摘要 目的:探讨脑电活动地形图(BEAM)对亚临床肝性脑病(SHE)的诊断价值。方法:对20例无临床肝性脑病的失代偿期肝硬化患者进行BEAM检查,并与33例正常人作对照分析。结果:20例肝硬化患者中,BEAM正常5例,轻度异常5例,轻一中度异常5例,中度异常5例,总异常率为75.00%(15/20)。BEAM异常的分布特点以弥漫性分布为主要表现,主要为慢频域(δ、θ、δ+θ/α+β)活动增强,Z值增高,只有3例波及α频域。结论SHE患者BEAM检查异常率明显增高,可作为SHE的早期诊断依据,由于该技术先进,无损伤且敏感,是诊断SHE的一种理想方法。本文认为BEAM检查中度以上异常者,可考虑进行预防性肝性脑病的药物治疗。 Aims The purpose of this study was to investigate the prognostic value of Brain Electrical Activity Mapping(BEAM) in sub clinical hepatic encephalopathy(SHE).Methods With BEAM,we checked 20 cases of Cirrhosis patients who had no compensatory period of SHE and 33 cases normal controls.Results The BEAM results showed that,among these patients,5 cases were nomal,5 cases slightly abnomal,5 cases slighily moderately abnomal and 5 cases moderately abnormal,with a total abnormity rate of 75.00%(15/20).The distribution of BEAM abnomity was charicteristic by diffused predominance,presenting an intensified activity and increased Z value in slowfrequency bands(delta,theta,the ratio of delta plus theta and alpha plus bata),with an exception of 3 cases whose abnomity waves spreading to alpha frequency band.Conclusions All this suggested that the marked increase of BEAM abnormity can be regarded as an early diagnostic evidence for SHE patients.Being sensitive and causing no injury,advanced BEAM is an ideal technique in SHE diagnosis.For those with moderately abnormity in BEAM examination,pharmaceutical treatmennt can be taken into account in SHE prevention.
