婴幼儿血清PICP和尿DPD的变化与生长发育的关系 被引量:3

摘要 【目的】探讨骨Ⅰ型胶原形成标志物Ⅰ型前胶原羧基端前肽(carboxyterminal propeptide of type I procol-lagen,PICP)和分解标志物脱氧吡啶啉(deoxypyridinoline,DPD)与婴幼儿生长发育的关系。【方法】用酶免法测定68例健康婴幼儿(婴儿31例、幼儿37例)血清PICP和尿DPD水平,用苦味酸不除蛋白法测定尿肌酐(creatinine,Cr),然后分别计算校正的尿DPD浓度(尿DPD/Cr)和血清PICP与尿DPD/Cr的差值(血清PICP-尿DPD/Cr)。分析其与月龄、身长、体重及体质指数(body mass index,BMI)的关系。【结果】两组内男女间身长、体重、BMI和骨代谢指标差异无显著性。婴、幼儿组间血清PICP、血清PICP-尿DPD/Cr差异有非常显著性,尿DPD/Cr差异无显著性。血清PICP、尿DPD/Cr与年龄、体重、身长呈负相关,与BMI无相关性,血清PICP-尿DPD/Cr则与年龄、体重、身长呈负相关,与BMI呈正相关。【结论】血清PICP和尿DPD分别是反映骨合成和骨代谢的良好生化指标;二者的差值是监测婴幼儿生长情况的较好指标。 [Objective] To investigate the variation of serum carboxyterminal propeptide of typeⅠ procollagen (PICP), urine deoxypyridinoline (DPD) and their relationships with sex,age, body weight, body length,and body mass index in infants and toddlers. [Methods] Sixty-eight healthy children (31 infants and 37 toddlers) were studied. Serum PICP and urine DPD were measured by enzyme immunoassay. The urine creatinine (Cr) were determined by carbazotic acid method. The urine DPD level was presented by corrected DPD(DPD/Cr). The difference of serum PICP and urine DPD/Cr were calculated. Just before the samples were collected, the children's body weight, body length were measured and the body mass index(BMI) were calculated. Their sex and age in months were recorded. [Results] The difference of age, body length, body weight,BMI,serum PICP and urine DPD/Cr between different sex in both groups was not significant. The difference of serum PICP and serum PICP-urine DPD/Cr between infants and toddlers group was significant, but the urine DPD/Cr was not. Both serum PICP and urine DPD/Cr had a negative correlation with age, body weight and body length, but had no significant correlation with BMI. Serum PICP-urine DPD/Cr had a negative correlation with age, body weight and body length, but had a positive correlation with BMI. [Conclusions] In infants and toddlers, the serum PICP and urine DPD are good specific biochemical indicator of bone formation or bone resorption. The difference of serum PICP and urine DPD/Cr could be better indicator for growth and development.
