摘要
【目的】了解糖尿病母亲所生新生儿的患病情况,母亲妊娠期糖代谢紊乱程度与严重新生儿疾病的关系。【方法】分析糖尿病母亲所生新生儿代谢性疾病、体格发育和器官成熟异常及其他相关疾病患病情况,比较糖耐量减低、妊娠期糖尿病及血糖控制情况与发复发作性低血糖、心肌肥厚、脑发育不成熟及严重脑损伤等严重新生儿疾病的关系。【结果】糖尿病母亲所分娩的新生儿中低血糖86例,检出率高达31.5%;大于胎龄儿88例,为31.8%,脑损伤38例,为13.7%;脑发育不成熟23例,检出率为8.3%。妊娠期糖尿病组严重新生儿疾病的检出率高于糖耐量减低组,母亲妊娠期血糖控制不满意组严重新生儿疾病检出率明显高于满意组(P<0.05)。【结论】妊娠期糖代谢紊乱可引起多种新生儿疾病,严重新生儿疾病与母亲血糖控制及糖尿病程度有关;脑发育不成熟和脑损伤值得关注。
[Objective] To investigate the incidence of infants of diabetic mothers disease and the relationship between degree of abnormal glucose metabolism during pregnancy and severe neonatal disease. [Methods] The metabolic disease,physical development, organic mature and other diseases of infants of diabetic mothers were analyzed. And the association of the degree of gestational impaired glucose tolerance , gestational diabetes mellitus, blood glucose control with the incidence of severe neonatal diseases such as frequent hypoglycemia, cardiomyohypertrophy,brain immaturities, severe brain injury and so on were also investigated. [Results] Among the 277 infants of diabetic mothers,eighty-six cases of hypoglycemia（31.5% ）, eighty-eight cases of large for gestational age（31.8%）, thirty-eight cases of severe brain injury （13.7%）, twenty-three cases of brain immaturities（8.3 %） were obsereved in the study. The checked rate of severe neonatal diseases in the gestational diabetes mellitus group was higher than those in the group of gestational impaired glucose tolerance. Furthermore, the checked rate of severe neonatal diseases in the group of unsatisfactory maternal blood glucose control were significantly higher than those in the group of satisfactory maternal blood glucose control （P〈0.05）. [Conclusion] Mother＇s abnormal glucose metabolism during pregnancy can cause all kinds of diseases in their infants. The degree of severe neonatal diseases closely related to the degree of maternal blood glucose control and diabetes during pregnancy. Brain immaturities and brain injury should be paid much attention on the infants of diabetic mothers.
