摘要 目的:回顾近年来利用胚胎干细胞治疗糖尿病的实验研究现状,认识诱导分化的胚胎干细胞移植治疗糖尿病的实验研究新进展及应用于临床前尚需解决的难题。资料来源:应用计算机检索Pubmed2000-01/2007-05胚胎干细胞治疗糖尿病的相关文章,检索词为“embryonic Stem cells,insulin-producing cells,diabetes”,并限定文献语言种类为English;以“胚胎干细胞,胰岛素分泌细胞,糖尿病”为检索词,检索中国全文期刊数据库2001-01/2007-01相关文章。资料选择:对资料进行初审,选取包括胚胎干细胞诱导分化与糖尿病的相关文献,开始查找全文。纳入标准:①胚胎干细胞诱导分化为胰岛素分泌细胞。②胚胎干细胞诱导分化与糖尿病治疗。排除标准:综述文献、重复研究、Meta分析类文章。资料提炼:共收集到44篇关于胚胎干细胞诱导分化为胰岛素分泌细胞治疗糖尿病的英文文献,纳入30篇符合标准的文献;8篇中文文献,纳入3篇进行探讨。资料综合:胚胎干细胞因具有独特的生物学特性有望成为糖尿病细胞移植最有前途的来源,体外可诱导胚胎干细胞分化为具备β细胞特征的功能性胰岛素分泌细胞,能使模型小鼠的高血糖恢复正常。但人类胚胎干细胞真正应用于临床前仍存在许多难题,如分化效率低、细胞不够成熟、免疫排斥、远期疗效等。随着基础研究的深入和临床应用难题的解决,胚胎干细胞将成为细胞移植治疗糖尿病最有希望的来源。结论:胚胎干细胞的应用将为细胞移植治疗糖尿病带来新的希望。 OBJECTIVE: To review the empirical study actualities of diabetes therapy with embryonic stem cells (ESCs) in recent years, to recognize the latest progresses got in the field of utilizing differentiated ESCs transplantation for diabetes in lab and problems should be resolved before clinical application. DATA SOURCES: The relevant articles on ESCs transplantation for diabetes from January 2000 to May 2007 were searched in Pubmed database with the key words of “embryonic stem cells, insulin-producing cells, diabetes” in English. Meanwhile, we searched the China Journal Full-text Database (CJFD) for articles in Chinese from January 2001 to January 2007 using the key words of “embryonic stem ceils, insulin-producing cells, diabetes”. STUDY SELECTION: The literatures on differentiated ESCs and diabetes were primary selection, and then full-texts were retrieved. Inclusive criteria were ①articles on inducing ESCs differentiated into insuling-producing cells and articles on ESCs differentiation and diabetes therapy. Exclusive criteria were reviews, repetitive researches and Meta analytical articles. DATA EXTRACTION: Totally 44 articles in English on differentiated ESCs transplantation and diabetes were collected in which 30 papers met the inclusive criteria, and 8 articles in Chinese were collected in which 3 papers were adopted to explore accorded with criteria. DATA SYNTHESIS: ESCs would become the new source of cell-transplantation for diabetes because of their inimitable biological traits. In vitro, ESCs could be induced into functional insulin-secreting cells which had traits of β cells, and could restore the hyperglycaemia of diabetes mouse. Many difficult problems existed in clinical utilization of human ESCs, such as low differentiating efficacy, immature cells, immunologic rejection, prospective efficacy and so on. With deeper study and these problems resolved, human ESCs would become the most promising source of cell transplantation for diabetes. CONCLUSION: The application of ESCs will brina new hope for treatina diabetes with cell transplantation.
