摘要
为观察气管吻合口修复的形态学改变,将24条犬气管分单纯横断、袖状切除和管型重建3组。术后4周各组吻合口上皮均基本修复。袖状切除组因吻合口张力大、局部缺血、乏氧导致瘢痕形成,出现不同程度的吻合口狭窄;管型重建组因吻合口张力小,基本不影响吻合口愈合,不易产生物合口狭窄。作者认为带肋间血管蒂胸壁复合软组织瓣管型是较理想的气管替代物。
This study is undertaken to observe the morphological changes in the healing of canine traheal anastomosis.Twenty-four dogs were divided into 3 groups:group I(n=8):simple transection;group II(n=8) slevae resection andgroup III(n=8):tubular reconstruction with chest wall soft tissues consisting of intercostal muscles parietal pleura and inter-costal vascular pedicle.An intraluminal elastic stet was used.At the 4th postoperative week,the anastomotic healing was seenin all groups.Owing to local hypoxia,ischemia and scar formation caused by excessive anastomotic tension,various degree ofanastomotic stenosis occurred in the sleeve resection group.In the reconstruction group,the healing was smooth because oflow-anastomotic tension,and stenosis rarely occurred.It is suggested that a composite chest,wall soft-tissue flap tube withan intercostal vascular pedicle might be an ideal tracheal substitute.
