摘要
目的探讨深入图像血氧分析在室间隔缺损术后评价肺氧摄取、血氧运输、组织氧利用中的价值。方法采用深入图像血气分析方法对50例室间隔缺损肺高压(观察组)和50例室间隔缺损无肺高压(对照组)的术后6,12,24 h行动脉血气分析,比较两组术后的深入图像血氧值。结果观察组与对照组比较,肺氧摄取和血氧运输无显著统计学差异(P>0.05),但组织氧利用差(P<0.01),室间隔缺损肺高压组术后24 h与术后2,4 h比较肺氧摄取值更低(P<0.01)。结论深入图像血气分析可提供肺氧摄取、血氧运输、组织氧利用等指标,反映出室间隔缺损肺高压术后在氧分压正常的情况下组织氧利用不良,较传统血气分析有明显优势。
Objective To study oxygen uptake in lungs, oxygen transport in blood and oxygen release into tissue in the postoperative patients of ventricular septal defect ( VSD ) with pulmonary hypertension with deep picture blood gas analysis, in order to redress the judgement of oxygen status through PaO2 and SaO2. Methods Arterial deep picture blood gas data of 50 VSD patients with nonpulmonary hypertension ( Group Ⅰ) and 50 VSD patients with pulmonary hypertension ( Group Ⅱ) was compared respectively at 2, 12 and 24 hours after operation. Results There were no significant differences between the two groups in oxygen uptake in the lungs and oxygen transport in the blood ( 〉 0.05 ) , except oxygen release into the tissue. In Group Ⅱ, the 24-hour group had lower oxygen release into the tissue than 2 - and 12-hour groups. Conclusion The deep picture blood gas analyzer may offer the important information on oxygen uptake, transport and release. It can reflect that the release of oxygen to the tissue may be poor even though a patient has a normal PaO2.
