靶控异丙酚复合不同剂量瑞芬太尼对成人气管插管血流动力学和脑电双频指数的影响 被引量:11

摘要 目的:观察靶控异丙酚复合不同剂量瑞芬太尼对成人气管插管血流动力学和脑电双频指数(bispectral index,BIS)的影响。方法:选择需全麻的择期手术成人患者40例,按照气管插管前诱导时使用不同剂量的瑞芬太尼,分为4组,每组10例。麻醉诱导用咪唑安定0.05mg/kg,异丙酚血浆靶控输注,靶浓度2.0μg/mL,维库溴胺0.1mg/kg,并分别缓慢推注不同剂量的瑞芬太尼0.5μg/kg(R0.5组)、1.0μg/kg(R1.0组)、1.5μg/kg(R1.5组)、2.0μg/kg(R2.0组),气管插管后仍以异丙酚血浆靶浓度2.0μg/mL、瑞芬太尼0.1μg/(kg·min)速度维持10min。观察记录基础值(t1)、麻醉诱导后插管前即刻(t2)和插管后2min(t3)、5min(t4)、10min(t5)各时点插管前后血流动力学及BIS值的变化、不良反应出现情况。结果:(1)各组BIS相应时间点与其t1比较均明显降低(P<0.05)。(2)各组t3与t1相应时间点的MAP、HR值比较,R0.5组MAP与HR均增高,R1.5组HR降低,R2.0组MAP与HR均降低(P<0.05)。(3)各组t3与其t2对应值比较,R0.5组、R1.0组、R1.5组MAP、HR及BIS值均增加,R2.0组仅HR增加(P<0.05)。结论:全麻诱导,异丙酚以2.0μg/mL血浆靶控浓度,合用1.0、1.5μg/kg瑞芬太尼时,血流动力学较平稳,但不能完全消除插管的刺激。合用2.0μg/kg瑞芬太尼时,可基本抑制气管插管反应,但副作用较多。
