内镜下睫状体光凝术的实验研究

摘要 目的探讨眼科内镜下二极管激光睫状体光凝术对眼压的影响及眼的组织学改变。方法将45只青紫蓝兔分成4组进行内镜下光凝,第1、2、3组术后观察眼压,在保持每个睫状突光凝程度一致的条件下,光凝不同的范围,依次为45°、90°和135°,术后观察眼压的变化。第4组做组织学检查。结果光凝术后,各组眼压均下降。经观察,第1组眼压又逐渐增高;第2组保持平稳;第3组出现过低的眼压。组织学检查,光凝后即刻睫状突上皮肿胀,基质血管扩张、充血。7d时,睫状突上皮大部分脱失,残存的上皮内以及上皮之间有较多囊肿。21d时,无色素上皮缺失或变薄,睫状突间质疏松程度减轻。光凝后3月,睫状体组织明显致密,有形成分减少。结论内镜下二极管激光睫状体光凝可以有效地破坏兔眼睫状突结构,光凝90°范围可以保持降压效果的稳定。 Objective The aim of this study is to analyse the result of intraocular pressures (IOPs) and report histopathologic changes in ciliary processes, following endoscopic cyclophotocoagulation (ECT) with diode laser was performed in eyes of the pigmented rabbits. Methods 45 pigmented rabbits were randomly divided into 4 groups, cyclophotocoagulation with endoscopic visualization was performed on each group. In Group 1,2 and 3, laser applications were kept in same effect of each ciliary process, but different in the area among groups, Area was set at an arc of 45° ,90° and 135°, respectively. ,The resulting effects of IOP were imaged in this three groups. In the Group 4, histologic examinations were performed. Results After the endolaser photocoagulation was performed, there was a decrease in the mean IOPs of each group. With increasing duration, the pressures in the treated eyes varied respectively, The final intraocular pressure in Group 1 was higher than it in the early time after treatment, in Group 3 was lower and it kept steady in Group 2. Pathological changes: it included ciliary epithelia thinned, bubble formation and vascular congestion of the stroma immediately after the procedure; 7 days later, ciliary epithelia were almost disrupted. There were cysts of the pigmented and nonpigmented ciliary epithelia. Stromal atrophy and vascular stricture were evident. 21 days after endolaser with cyclophotocoagulation, disruption of the ciliary epithelium was seen. The nonpigmented epithelium was collapsed or thinned, the pigment clumped and stromal edema was alleviated. Light micrographs of chronic diode laser lesions of the ciliary body were noted 3 months after laser treatment. Processes are shortened, with fusion occurred in the ciliary body. The degree of damage was marked. Conclusions ECT with diode laser can provide effective Control of IOP in the pigmented rabbits. At exposures of 90° , the effect of lowing IOP is steady. With a low rate of severe surgical complications, endoscopic cyclophotocoagulation is a safe and effectual method for lowering IOP in the pigmented eyes.
