摘要
目的探讨慢性胰腺炎的CT灌注特点。资料与方法使用16层螺旋CT对8例慢性胰腺炎患者(慢性胰腺炎组)和71例正常对照者(对照组)进行CT灌注扫描(后处理由Philips公司提供),由两名高年资影像科医师对图像进行分析。计算各组的组织血流量(BF)、组织血容量(BV)及平均通过时间(MTT)值并采用t检验统计分析各组间灌注数据的差异。结果(1)慢性胰腺炎组胰腺头部、体部及尾部的BF值分别为(27.6±14.0)ml·100g-1·min-1、(33.1±19.8)ml·100g-1·min-1、(47.1±13.0)ml·100g-1·min-1;BV值分别为(23.8±0.9)ml/100g、(26.1±7.6)ml/100g、(24.3±6.7)ml/100g。而对照组的BF分别为(63.7±22.1)ml·100g-1·min-1、(72.6±23.3)ml·100g-1·min-1、(72.0±20.1)ml·100g-1·min-1;BV值分别为(35.8±11.8)ml/100g、(35.6±15.2)ml/100g、(35.2±11.9)ml/100g。统计学分析显示胰腺各部的灌注参数(BF、BV)与对照组比较,均有明显的降低(P值均<0.01);MTT的变化差异无统计学意义。(2)双肾、脾脏的BV值、BF值和肝脏的门静脉灌注(PVP)值与对照组比较,差异无统计学意义。结论CT灌注成像能定量分析胰腺及其周围重要器官的血流灌注特点。慢性胰腺炎患者的胰腺血流灌注显著降低,而相邻的双肾、肝脏及脾脏的血流灌注没有明显异常。
Objective To explore the CT perfusion features in chronic pancreatitis (CP). Materials and Methods 16 shces spiral CT perfusion (the CT perfusion analysis software offered by PHILIPS) was applied to 8 patients with chronic pancreafitis( CP group) and 71 patients without pancreas disease( control group). CT images were interpreted by two senior radiologists. The BF values,BV values and MTT values were measured and statistic analyzed. T test was used to analyze the difference of blood perfusion between normal pancreas and CP. Results ( 1 ) in CP group, at the head of pancreas, BF was (27.6 ±14.0) ml·100g·min, BV was (23.8 ±0.9)ml/100g, MTr was (71.8 ±49.5) s; at the body of pancreas, BF was(33.1 ± 19.8) ml· 100g· min, BV was(26.1 ± 7.6) ml/100g, MTr was (69.6 ±46.3) s; at the tail of pancreas, BF was (47.1 ± 13. 0)ml· 100g· min, BV was (24.3±6.7 ) ml/100g, MTT was (34, 6 ± 17. 0)s. The statistic significance was found for BV and BF in all parts of pancreas compared CP group with control group (P 〈0. 01 ) , but the MTT had not statistic significance (P 〉 0. 05). ( 2 ) No statistic significance was found for BV or BF of spleen and kidneys or PVP of liver compared CP group with control group( P 〉 0. 05). Conclusion The CT perfusion is a useful tool for quantitative analysis blood perfusion characteristic of pancreas and important organs around it. The blood perfusion is decreased in pancreas with CP, but no change of blood perfusion is found in surrounding organs like kidneys, spleen and liver.
