摘要
目的探讨经皮椎体成形术中骨水泥外渗原因及防范方法。方法经皮椎体成形术37例,57个椎体。椎体肿瘤15例,其中转移性肿瘤13例,椎体血管瘤2例;骨质疏松性压缩性骨折15例;创伤性压缩性骨折7例。结果22个椎体出现骨水泥外渗,其中渗入椎管内1例,椎间隙3例,椎体侧方1例,穿刺部位软组织3例,椎旁静脉14例。2例出现下肢神经症状,术后短期内恢复。随访1-6个月,患者对治疗结果均满意。结论椎体成形术是治疗某些椎体病损的有效方法。避免骨水泥渗漏的措施包括术前确认椎体后缘是否完整,注意穿刺技术、操作程序、注入骨水泥的量和粘稠度。
Objective To explore the causes and prevertions of bone cement leaking in percutaneous vertebroplasty. Methods Thirty-seven cases of vertebral body diseases with 57 vertebra percutaneous vertebroplasty procedures were treated. There were 15 cases of vertebral tumour, 15 osteoporotic vertebral compression fractures(VCF) and 7 traumatic VCF. Results The bone cement leaked in 22 vertebraes with 1 case in the vertebral canal, 3 in the vertebral space, 1 on the side of the vertebrae, 3 at the site of the puncture and the other in the veins. Two cases had a neural roots symptom and recovered in a short period of time postoperatively. The treating results were satisfactory during the first 6 months of follow-up. Conclusion The percutaneous vertebroplasty is an effective method in the treatment of pain caused by vertebral infirmities. The safety measures to reduce the risk of cement extravasation include the integrality of posterior edge of the vertebra, puncture technique, manipulation procedure and the amount and consistency of bone cements to be injected.
