摘要
目的探讨99Tcm-亚甲基二磷酸盐(99Tcm-MDP)乳腺及全身骨显像对乳腺肿块良恶性的鉴别及早期发现骨转移的临床价值。方法66例乳腺肿块患者均静脉注射99mTc-MDP20-25mCi后进行15分钟乳腺显像及3小时全身骨显像,取乳腺平面俯卧左、右侧位和仰卧前位显像。病理检查均在乳腺显像后10d内进行。结果99Tcm-MDP诊断原发性乳腺癌的灵敏度为92.9%(39/42),特异性为91.7%(22/24),阳性预测值为95.1%(39/41),阴性预测值为88.0%(22/25)。同时,全身骨显像有9例阳性,提示骨转移。结论99Tcm-MDP乳腺及全身骨显像是一种简便、有效而准确的非创伤性鉴别乳腺肿块良恶性的方法,对乳腺癌的诊断、良恶性乳腺肿块的鉴别诊断及早期发现骨转移灶均具有重要临床价值。
Objective To evaluate the clinical value of 99Tcm MDP breast and bone imaging detecting malignancy in breast neoplasma and earlybone metastasis. Methods Sixty-six women referred for a suspicious breast lesion were studied with ^99Tc^m-MDP breast and bone imaging. Using planar prone imaging performed 15 rain and 3 hours postinjection of 20-- 25 mCi ^99Tc^m-MDP. Three planar views right and left lateral prone and anterior supine thoracic views, were obtained. Excisional breast biopsy was performed within 10 clays after scintimammography. Results The sensitivity of scintimammography for detecting primary breast cancer was 92.9 % (39 true-positive, 3false-negative)and the Specificity was91.7%(22 true-negative,2 false-positive,the positive predictive value was 95.1%and the negative predictive value was 88.0%. There were 9 positive results in bone imaging showed bone metastasis. Conclusions ^99Tc^m-MDP breast and bone imaging is a simple, effective and accurate method for differdntiating the malignant breast neoplasma from benign ones no invasively. It is of great value in differential diagnosis of benign and malignant masses of the breast and in demonstrating early latent bone metastasis.
