高强度军事训练引起急性肾功衰竭11例分析 被引量:9

摘要 目的:总结高强度军训所致ARF患者的临床病理特点。方法:回顾性分析11例高强度军训后发生ARF患者的临床及病理表现。均为青年男性,年龄19—32岁,起病前有明确的3—10km超负荷军训史。结果:所有患者均有恶心、呕吐,血压升高者7例,少尿和无尿者5例,晕厥3例。辅助检查:11例有轻-中度蛋白尿,9例尿NAG酶升高,血肌酐170-1720μmoL/L,6例ALT和AST升高,无贫血,无双肾缩小。肾组织病理为肾小管坏死脱落,间质水肿,间质内大量红细胞淤积。治疗后所有患者肾功能恢复正常。结论:高强度军事训练可引起急性肾功能衰竭,临床为少尿性和/或非少尿性,肾活检病理为急性小管间质病变,预后良好。 Objective: To analyze the clinical and pathological characteristic of the patients with acute renal failure induced by overload military training. Methods: The clinical and pathological data of 11 patients admitted to our hospital from 2000 to 2005 were reviewed. Results: The average age of 11 patients was (25.3 ± 5.3) years old. The clinical manifestatation included proteinuria ( 100% ), increased urinary N - acetyl -β - D - glucosaminidase (NAG) (81.8%), hypertension (63.6%), oliguria (45.5%), nausea and vomitus. Kidney biopsy specimens were obtained from these patients. The main pathological changes were acute tubular necrosis and edema of the interstitial space. Conclusion: Overload military training can lead to ARF, causing oliguria, proteinuria and hypertension. The primary histologic lesion is acute tubuli- interstitial nephritis.
