急症外科治疗晚发性维生素k缺乏症所致颅内出血疗效的初步探讨 被引量:3

Emergency Surgical Treatment of Intracranial Hemorrhage due to Delayed Vitamin-K Deficiency:A Preliminary Study of Therapeutic Effectiveness

摘要 以自发性颅内出血为特征的晚发性维生素长缺乏症是一种对婴儿生命危胁性较大,病死率高,幸存者易致残的疾病.但又是一种完全可以预防的疾病.本文报道48例,男:女2.2:1.具备足月儿,体重超过3000g,单纯母乳喂养儿3种情况者占95,83%.直、间接胆红素增高,伴或不件肝功能损害占68,75%.本组患儿经头颅CT证实,单纯硬脑膜下腔出血占31.25%,硬脑膜下腔出血伴其他颅内出血占31,25%,硬脑膜外出血占3.25%,过去采取内科保守治疗,现对以上两类颅内出血及时做颅内钻孔引流,术前静脉注射1剂维生素长。 Delayed vitamin-K deficiency in infants is a life-threatening disease with a high fatality rate and a tendency to resulting in disability in the survivors, but it is completely preventable. This article reported 48 patients (male:fermale=2.2:1) among whom the full-term infants, infants with their body weight over 3000 gm and simply breast-feeding infanis all accounted for 95.83%. The infants with increased direct and indirect bilirubin and with or without liver dysfunction accounted for 68.75%. Cranial CT scan revealed that simple subdural hemorrhage accounted for 31.25%, subdural hemorrhage plus other intracranial hemorrhage accounted for 31.25% and epidural hemorrhage accounted for 3.25%, As compared with the conservative treatment in the past, better therapeutic effectiveness was seen in the current treatments of both typs of intracranial hemorrhage. with timely skull trepanation and drainage of hematocele, preoperative injection of vitamin-K, and intraoperative or postoperative transfusion of plasma or fresh blood.
