摘要
目的:探讨畸变产物耳声发射(distortion product acoustic emission,DPOAE)用于新生儿听力筛查的最佳时机。方法:对我院产科病房3个月内出生的100例新生儿(200耳)进行DPOAE测试。测试前详细登记新生儿及父母的自然状况。测试在新生儿处于安静状态下进行。对每1名受试对象进行9次DPOAE测试,测试时机分别为出生后1、2、3、4、5、6、7、15及第30天。应用SPSS统计软件进行统计学处理。结果:DPOAE测试通过耳数及通过率随着日龄的增加而上升。第1天通过耳数接近一半(47%),第4天达到了90%以上。②在DPOAE通过率-日龄曲线中,前3个时间点处的斜率较大,波形较陡。曲线在第4天以后相对平缓,DPOAE通过率趋于饱和。③在相邻日龄新生儿DPOAE通过率的比较中,前3组的P值均<0.05,通过率有显著性差异,后4组的P值均>0.05,通过率差异无统计学意义。结论:DPOAE测试非常适合于大规模的新生儿听力筛查,尤其是初筛。②DPOAE测试结果尚不足以做为金标准进行听力学诊断,必要时需结合ABR及声阻抗等检查。③DPOAE测试用于新生儿听力筛查的最佳时机为不应少于出生后4天。
Objective: To find the best time in DPOAE test when it is used in auditory screening of newborns. Methods: 100 newbores (200 ears), in three months, from gynecotokological ward of this hospital were examined in auditory room by GS1-70 -OAE when they were calm. Circumstances of newborns and their parents were registered in detail. Every one was examined for 9 times namely 1, 2, 3, 4, 5, 6, 7, 15 and 30 after born in quiet situation. The results were processed by SPSS. Results: (1)The passing rates of DPOAE rose by days. Through approximate half in 1 st days, it approached over 90% in 4th day. (2)The curve of passing rate -day was steep on the 1 st, 2nd and 3rd points, and the later section was pretty flat. (3)Comparing the adjoining passing rates, it was found that " P" of the fn'st 3 groups were less than 0. 05, and there were differences in it. But the later was reverse. Conclusion: (1)DPOAE test is very fit for auditory screening of newborns, especially for the first screening. (2)DPOAE test can not be used as a golden level in clinical auditory diagnosis, and should be combined with ABR and acoustic immittanee measurement when necessary. (3)The best time to carry DPOAE test in auditory screening of newborns is not earlier than the fourth day after born.
