摘要
目的观察激光氧液治疗新生儿缺血缺氧性脑病(hypoxic-ischemic encephalopathy,HIE)的临床疗效及护理。方法选择106例HIE患儿,将其随机分为治疗组(54例)和对照组(52例),对照组给予常规治疗,治疗组在常规治疗基础上,用DJF-IA型多功能激光血液治疗仪,以5%葡萄糖5~10 ml/kg做载体,置于血疗仪中,接受He-Ne激光照射15 min并充氧,加用东莨菪碱0.01~0.05 mg/kg,静脉滴注1次/d,疗程10 d。结果治疗组54例患者中,显效41例,有效10例,总有效率为94.45%;对照组52例,显效10例,有效21例,总有效率为59.61%,两组患者比较有统计学意义(P<0.01)。治疗组患者治疗后的PaO2、SaO2明显升高,和治疗前比较有统计学意义(P<0.05),PaCO2较治疗前下降,比较有统计学意义(P<0.05);治疗后患者的pH值较治疗前升高,比较无统计学意义(P>0.05)。结论激光氧液加东莨菪碱治疗HIE患儿疗效好,疗法安全,无不良反应。同时,保持呼吸道通畅,正确氧疗及护理是治疗成功的关键。
Objective To observe the curative effect of Laser Oxygenate Fluid and Scopolamine (LOFS) in treating Neonatal Hypoxic-Ischemic Encephalopathy (HIE) and the related nursing. Methods A total of 106 HIE patients were randomly divided into treatment group ( n =54) and control group( n =52). All patients were given routine treatment and those in the treatment group were also treated by muhifunctionallaser radiation hemotherapeutic instrument. 5% GS (5- 10 ml · kg ^-1) were used as vectors for radiation (15 min) with He-Ne laser and oxygenation. Scopolamine (0.01-0.05 mg · kg^-1) was given intravenously daily for 10 days. Results Forty one of the 54 patients in the treatment group had good outcome and 10 had a moderate outcome, with the total effective rate of 94.45% ; 10 of the 52 patients in the control group had good outcomes and 21 had moderate outcomes, with a total effective rate of 59.61%; and there was significant difference between the 2 groups( P 〈0.01). Patients in the treatment group had significantly increased PaO2, Sao2 and decreased PaCO2 after treatment ( P 〈 0.05). Conclusion LOFS is a safe treatment for HIE and has satisfactory outcomes, with no adverse reaction. The key to successful treatment is to keep airway unobstructed and timely oxidation and nursing.
