镁剂在麻醉中的作用 被引量:1

摘要 镁在细胞膜离子泵运转过程中起着重要的作用,它作为钙通道阻滞剂和钙通道活性的调节剂而发挥诸多的生理功能。血镁浓度升高能影响心肌的收缩性和心脏传导系统的兴奋性,并且抑制肾上腺素能神经末梢,肾上腺髓质和肾上腺素能节后交感神经纤维儿茶酚胺的释放,所以镁剂具有降压、减慢心率等对抗心血管兴奋性作用,抑制气管插管时的心血管反应,对血流动力学方面有稳定作用。在中枢神经系统,镁离子通过对钙通道的竞争性拮抗,调节突触前受体神经递质的释放,并通过非竞争性阻断兴奋性氨基酸受体NMDA而发挥作用。依据上述原理及大量研究报告,表明镁具有协同吸入性麻醉药、静脉麻醉药、麻醉性镇痛药及肌松药等作用,因此在临床麻醉及疼痛治疗等方面具有良好的应用前景。
