Reconstruction of both the anterior cruciate ligament and posterior cruciate ligament with ligament advanced reinforcement system artificial ligament transplantation under knee arthroscopy

摘要 目的探讨关节镜下LARS(Ligament Advanced Reinforcement System,LARS)人工韧带同时重建前(An-terior cruciate ligament,ACL)、后交叉韧带(posterior cruciate ligament,PCL)的方法和临床疗效。方法从2006年1月至2007年4月,用LARS人工韧带同时重建ACL、PCL2例。应用Lysholm功能评分表评估膝关节功能,采用抽屉实验检查膝关节前后松弛度。结果2例均获得随访,随访时间分别为8月、3月。2例患者术后患膝关节不稳定症状消失,胫骨后坠征阴性,前、后抽屉试验阴性。膝关节功能评估采用Lysholm功能评分标准,术前平均分别为45.6±7.6分,术后平均为80.3±9.1分。结论关节镜下应用LARS人工韧带同时重建ACL、PCL可更好地恢复膝关节的稳定性,且创伤小,并发症少,近期疗效满意,但远期效果仍需进一步观察。 Objiective To discuss the operation and clinical effect of the reconstruction of both the anterior cruciate liga- ment(ACL) and posterior cruciate ligament(PCL) with ligament advanced reinforcement system(LARS) artificial liga- ment transplantation under knee arthroscopy. Methods From January 2006 to April 2007, 2 patients with both ACL and PCL injury were treated simultaneously by reconstruction with LARS artificial ligament transplantation. Lysholm knee score scale was used for functional evaluation and drawer test for stability evaluation. Result Two cases were available at follow-up, the average duration of follow-up were 8 and 3 months respectively. After operation, the instability smptom of knee joint was disappear, and the sag sign and posterior drawer test were negative in 2 cases. The patients were evaluated according to Lysholm knee score scale, the mean scores was 45.6±7.6 and 80.3±9.1 before and after operation respectively. Conclusion Under knee arthroscopy, the reconstruction of both ACL and PCL with LARS artificial ligament transplantation can regain stabilization of knee joint, and it can result in limited invasion ,few complication and satisfactory clinical results.
