分水岭脑梗死与脑动脉狭窄的CTA研究 被引量:2

摘要 目的观察分水岭脑梗死与脑动脉狭窄的关系及可能的发病机制。方法回顾性分析经CT或MRI证实的分水岭脑梗死18例患者的临床资料,将分水岭脑梗死分为皮层前型、皮层后型及皮层下型,利用CTA和颈动脉彩超检查判断颅内外大动脉有无狭窄或闭塞,观察分水岭脑梗死与脑动脉狭窄的关系。结果18例分水岭脑梗死患者均存在程度不等的颅内外大血管的狭窄,且以颅内大血管狭窄更为常见,大脑中动脉狭窄约72.2%,大脑后动脉狭窄66.7%,大脑前动脉狭窄约44.4%,椎动脉狭窄约38.9%,颈内动脉闭塞仅1例。结论亚洲人分水岭脑梗死以颅内大动脉狭窄常见,特别是大脑中动脉最常见,因此对严重的颅内大动脉狭窄患者进行内膜剥脱术或支架成形术治疗可能有利于预防分水岭脑梗死的发生及改善预后。
