摘要
[目的]探讨后路短节段椎弓根钉内固定加植骨融合矫正儿童先天性半椎体脊柱后凸畸形的可行性及短期疗效评价。[方法]对2001年11月~2006年1月挑选的13例先天性半椎体脊柱后凸儿童进行后路短节段椎弓根钉内固定加植骨融合畸形矫正术,男8例,女5例;平均10.4岁(5~13岁)。Cobb’s角平均为42.7°(28°~48°)。术后支具固定8个月。[结果]术后X线片随访12个月~5年3个月(平均2年3个月)。矢状面Cobb’s角由平均42.7°矫正至平均9.3°,畸形矫正率达到78.2%。随访过程中矫正的Cobb’s角平均增大1.6°,无椎弓根切割,无神经损伤,没有发生植骨不愈合的病例,没有患者出现侧凸畸形。[结论]后路短节段椎弓根钉内固定加植骨融合矫正儿童先天性脊柱后凸,手术创伤小,短期效果可靠,但应严格掌握手术指征。
[ Objective ] To discuss the feasibility of posterior short segment fixation with pedicle screws and bone grafting in the treatment of child congenital kyphosis caused by hemivertebra and evaluate the short-term outcomes of the procedure. [ Method] From November 2001 to January 2006, 13 children with congenital kyphosis caused by hemivetebrae were selected to undergo the operation of posterior short segment fixation with pedicle screws and bone grafting. There were 8 males and 5 females, the average age was 10. 4 years (5 - 13 years). [ Result] The patients were followed up with roentgenology for 12 months to 5 years and 3 months (average 2 years and 3 months). The Cobb's angles on sagital planes were averagely reduced from 42. 7^o to 9. 3^o the average rectifying rate was 78.2%. Through the following-up period, Cobb's angles increased only 1.6^o averagely, there was no loss of internal fixation, no nerve injury, no nonunion of grafted bone and there was no scoliosis occurring. [Conclusion] The surgery of posterior short segment fixation with pedicle screws and bone grafting in treatment of child congenital kyphosis can decrease injuries caused by the procedure and has good short-term outcomes. However, indications of the operation should be strictly controlled.
