摘要
目的探讨骨代谢转换生化指标在不同性别、年龄组的变化规律,为建立不同性别各年龄组的参考范围和开展骨质疏松症的预防、监测提供参考。方法对276例各年龄段非骨质疏松症人员,采用电化学发光免疫分析技术、放射免疫法以及动态速率法分别测定血清中Ⅰ型胶原C-末端肽(β-Crosslaps)、骨钙素和碱性磷酸酶的含量。结果碱性磷酸酶在男、女性中没有显示规律性变化。β-Crosslaps在男、女血清中的含量有随年龄增大而升高的趋势,与最低值年龄组比较,均只是到了50~59年龄组才开始有显著性差异(P<0.05),女性自此增幅明显,在组间开始有显著性差异(P<0.05),但男性在各年龄组间无显著性差异。女性骨钙素在50岁前,有随年龄增高而降低的趋势,至50~59岁突然明显升高,与前组比较有显著性差异(P<0.05),随后各组缓慢升高,组间不再有显著性差异;男性骨钙素在30~39组别最高,以后有随年龄增高而降低的趋势,降低幅度也较平缓稳定,各年龄组间无显著性差异(P>0.05)。β-Crosslaps/骨钙素比值在男女两组均有逐渐升高趋势。结论β-Crosslaps与骨钙素,尤其是二者的比值在男、女性别及各年龄组中有较好的变化趋势,对预防和治疗骨质疏松症有重要指导意义。
Objective To investigate the age-and gender-related changes in serum biochemical markers for bone metabolic turnover to establish the reference ranges for these markers for osteoporosis prevention and monitoring.Methods In 276 healthy individuals,serum type Ⅰ collagen C-telopeptide(β-Crosslaps),osteocalin and alkaline phosphatase levels were measured by electrochemiluminescence immunoassay,radioimmunoassay and consecutive monitoring,respectively.Results No significant changes in serum phosphatase was found in relation to age and gender.Serum β-crosslaps levels tended to increase with age in male and female subjects,but only until the age of 50-59 years,such changes began to be statistically significant(P〈0.05)as compared with the levels in the youngest group.Above this age range,β-crosslaps level in female subjects increased dramatically,showing significant difference between adjacent age groups(P〈0.05),which,however,was out observed in male subjects.Serum osteocalin level was lowered with age in female subjects before the age of 50 years,but a significant increase occurred at the age of 50-59 years(P〈0.05)followed by a relatively stable and mild increase that did not result in significant difference between adjacent age groups.In male subjects,serum osteocalin level reached the peak level at the age of 30-39 years followed by stable and mild decrease with age.The ratio of β-crosslaps/osteocalin tended to increase with age in both male and female subjects.Conclusion There are age-and gender-related changes in serum β-crosslaps,osteocalin and especially β-crosslaps/osteocalin ratio,suggesting the value of testing serum β-crosslaps and osteocalin in the prevention and treatment of osteoporosis.
