摘要
目的探讨免疫固定电泳技术在多发性骨髓瘤(MM)诊断中的价值。方法对70例多发性骨髓瘤的血清蛋白电泳和免疫固定电泳结果进行分析。结果70例MM血清蛋白电泳65例检出M-带,有5例出现极不明显的或不出现M-带,但在免疫固定电泳图谱中70例均可见与对应抗血清形成单克隆条带。5例血清蛋白电泳出现不明显条带或不出现条带的标本经免疫固定电泳分析2例为IgD-λ型,2例为轻链型,1例IgE—κ型。结论免疫固定电泳不仅能对M蛋白准确分型而且能提高单克隆IgD、IgE以及轻链型M-蛋白的检出率,是多发性骨髓瘤的诊断和鉴别诊断的重要手段。
Objective To investigate the value of immunofixation eletrophoresis technique in the diagnosis of multiple myeloma (MM). Methods The result of serum protein electrophoresis and immunofixation electrophoresis was analysed in 70 cases of multiple myeloma (MM). Results In 70 cases of MM serum protein electrophoresis, 65 cases of M-zone were detected, and the other 5 cases were detected with M-zone very unclear or no M-zone at all. But in the immunofixation electrophore- gram, 70 monoclonal zones formed by the correaponding anti-sera could be observed. The above 5 species were tested by immunofixation electrophoresis with 2 cases of IgD-λtype, 2 cases of light chain type and 1 case of IgE-κ type. Conclusion Immunofixation electrophoresis can not only exactly typing M-protein, but also increase the detection rate of monoclonal IgD, IgE and light chain type M-protein. It may be applied as a means to the diagnosis, treatment and prognosis evaluation of MM.
