摘要
目的探讨血清铁蛋白与2型糖尿病的关系。方法对80例2型糖尿病患者以及69例健康体检者分别测定身高、体重、腰围、臀围、血压;空腹血糖、口服75 g葡萄糖粉测量2小时血糖、空腹胰岛素、血清铁蛋白、总胆固醇、甘油三酯、高密度脂蛋白、低密度脂蛋白及C反应蛋白浓度。计算体重指数、腰臀围比及胰岛素抵抗指数。结果与对照组相比,初诊的2型糖尿病患者具有较高的BMI、WHR、FINS、TG、C-RP以及SBP水平(P<0.05);HOMA-IR明显升高(P<0.05);较低的HDL-C水平(P<0.05);而TC、LDL-C水平及DBP无明显差异(P>0.05);糖尿病组不论男性与女性SF浓度明显高于对照组(P<0.05)。通过直线相关分析可知,经对数转换后的2型糖尿病患者SF与TC、TG、LDL-C、C-RP呈正相关(P<0.05);与FBG、PBG、HOMR-IR及DBP显著正相关(P<0.01);与HDL-C呈负相关(P<0.05);与SBP、BMI、WHRF、INS非线性关系。经多因素逐步回归分析发现血清铁蛋白是参与HOMR-IR的独立变量。结论本研究证实2型糖尿病患者体内铁超负荷;而铁超负荷可能是导致胰岛素抵抗,促进糖尿病发生的许多重要的危险因素之一;并且血清铁蛋白可以预测糖尿病代谢控制程度。
Objective To study the relationship between ,serum ferrinfin and type 2 diabetes.Methods A total of 80 patients with 2 type diabetes and 69 healthy people were entered into study. All subjects were evaluated BMI (calculated as weight in kilograms divided by height in meters squared), waist-to-hip ratio (WHR), blood pressure, fasting, post-prandial glucose levels, serum HDL cholesteml,sertan triglycerides, serum ferritin and C-reactive protein. Insulin resistance was determined using the homeostasis model assessment (HOMA). Results Individuals with diabetes had higher systolic blood pressure, triglyceride concentrations, BMI, WHR, sermn C-reactive protein concentrations, and fasting insulin concentrations and had lower HDL cholesterol concentrations than individuals with normal glucose concentrations. Newly diagnosed diabetes had statistically higher ferritin concentrations among men and women. Log-transformed serum ferritin correlated positively with TC, TG, LDL-C, CRP, FBG, PBG and HOMA-IR, negatively with HDL-C, but not with BMI, WHR, SBP and FINS. Using multiple regression analysis, we found that SF also is independent variable. Conclusion Elevated iron stores precede the development of insulin resistance and contribute to the increased risk association with type 2 diabetes. Serum ferritin may also be a marker of ix)or metabolic control in the diabetic patients.
