摘要
目的探讨紫绀型及非紫绀型先天性心脏病心肺转流(CPB)前后心肌组织中血红素氧合酶表达及其变化的规律。方法紫绀型及非紫绀型先天性心脏病各15例,手术中分别于心肺转流前后取右房心肌标本,进行常规组织学检查,采用血红素氧合酶-1(HO-1)单克隆抗体进行免疫组织化学检查。所有病例手术前后检测心功能。结果CPB后两组心肌组织均轻度混浊肿胀,较CPB前改变不明显;HO-1在心肌组织中表达的部位以胞浆为主。CPB前紫绀型组HO-1积分光密度(5.55±2.32)较非紫绀型组(4.22±2.01)高(P<0.05),两组HO-1积分光密度均较CPB前增加,紫绀型组增加量(1.05±0.45)较非紫绀型组(3.22±0.67)低(P<0.01)。两组CPB后心输出量均下降。紫绀型组心输出量下降(1.94±0.55)L.min-1.m-2,较非紫绀型组的(1.05±0.39)L.min-1.m-2高(P<0.01),HO表达变化与心功能改变呈正相关。结论缺氧诱导HO-1表达增加,血红素氧合酶可能有保护心肌,改善心脏功能的作用。
To investigate the changes of heme oxyginase-1(HO-1)expression in human heart tissues from cyanotic and acyanotic congenital heart defect(CHD)patients.Methods Fifteen acyanotic CHD patients(group A)and 15 TOF patients(group B)were chosen in this study.Right atria tissue pieces were taken before and after cardiopulmonary bypass(CPB)during operation.Routine histological examinations were carried out and HO-1 expression was detected by immunohistochemical(IHC)staining using monoclone antibody.Heart function was determined by Doppler echocardiogram examination.Results Myocardial albuminous swelling was found in the two groups after CPB and no significant histological changes were found in myocardial tissue compared with that before CPB.HO-1 expressions were mainly located in cytoplasm.Before CPB,HO-1 accumulative optical density(OD)in cyanotic group B(5.55±2.32)was higher than that in group A(4.22±2.01).After CPB,HO-1 accumulative OD increased in the two groups.The OD increase in group B(1.05±0.45)was lower than that in group A(3.22±0.67)(P〈0.01).Cardiac output was decreased in the two groups.The decrease in cardiac output in group B(1.94±0.55)L·min^-1.m^-2was higher than that in group A [(1.05±0.39)L·min^-1.m^-2(P〈0.01).A positive relationship between HO-1 expression and heart function was found.Conclusion HO-1 expression is increased after anoxia induction.Heme oxygenase may have the effect of protecting myocardial injury and improving cardiac function.
