体内γ刀、体外高频热疗、化疗联合治疗晚期肺癌36例疗效观察 被引量:4

EFFECT OF BRACHYTHERAPY COMBINED WITH EXTERNAL HIGH FREQUENCY THERMOTHERAPY AND CHEMICAL THERAPY ON 36 PATIENTS WITH ADVANCED LUNG CANCER

摘要 目的观察CT引导下组织间植入^125I粒子联合体外高频热疗及化疗对晚期肺癌的疗效。方法患者入院后先行肺穿刺活检或经支气管镜活检以明确肿瘤病理类型,经TPS计划系统计划后行体内γ刀治疗。3d后行化疗(EP方案或顺铂+紫衫醇方案,每4周1次)及体外高频热疗(每周2次,共4周)。所有患者术后第1、2、3、6、9、12个月内分别行胸部CT检查随访。根据肿瘤变化将疗效分为4级。Ⅰ级,明显缓解(肿瘤缩小50%以上)。Ⅱ级,缓解(肿瘤缩小25%~50%)。Ⅲ级,轻度缓解(肿瘤缩小1%~25%)。Ⅳ级,无效(肿瘤无变化或增大,临床症状亦无缓解)。Ⅴ级,复发或转移。结果术后第1、2、3、6、9、12个月明显缓解,分别占38.9%、66.7%、77.8%、83.3%、83.3%和77.8%。结论体内γ刀、体外高频热疗、化疗联合治疗晚期肺癌是一种安全、可靠、疗效显著的治疗方法。 Objective To observe the curative effect of CT-guided interstitial ^125I particle implanting combined with external high frequency thermotherapy and chemical therapy on advanced lung cancer patients. Methods The needle biopsy or the bronchoscopic exanimation were made to identify the pathological category at frist. Then the patients received the internal gamma knife operation after planned by the TPS. The chemical therapy(EP program or cisplatin+paclitaxel program,once 4 weeks ) and external high frequency thermotherapy(twice a week for 4 weeks) were applied at 3 days after operation. All the patients took the following-up thoracic CT examination at 1,2,3,6,9,12 month after operation. According to the size of the tumor, the curative effect can be divided into 4 grades. Ⅰ grade,obvious relief(the tumor reduced more than 50% in size). Ⅱ grade,relief (tumor reduced 25%~50% in size). Ⅲ grade,slight relief(tumor reduced 1% ~ 25% in size). Ⅳ grade, no effect (tumor had no obvious change and clinic symptoms did not relieve).Ⅴ grade,recur or transfer. Results 1.2.3.6.9.12 month after the treatment,the rates of grade 1 relief were 38.9%.66.7%.77.8%.83.3%.83.3% and 77.8% respectively. Conclusion The internal gamma knife combined with external high frequency thermotherapy and chemical therapy is a safe,reliable and effective therapy method for advanced lung cancer.
