根尖诱导成形术后根尖封闭的种类及影响因素分析 被引量:25

摘要 目的本文研究根尖诱导成形术后根尖封闭种类及影响因素,以期对临床治疗提供指导。方法选择在本院门诊进行根尖诱导成形术后根尖完全封闭并进行根管治疗的75颗牙齿,完整记录临床情况,发育程度,根尖周病变情况等。利用扫描仪保存术前,中,后X线牙片,以对侧同名牙齿发育情况为对照牙,详细观察并记录根尖封闭种类,并以封闭种类为自变量通过多元回归分析影响牙根尖封闭种类的因素。结果根尖诱导成形术后的根尖发育与对照牙一致的仅占25%。牙根虽然发育但短于对照牙或者牙根未发育根尖硬组织封闭成形这两类占多数为75%。多元回归分析发现根尖周病变是影响牙根尖成形的主要因素。有病变的牙齿诱导后牙根发育短,硬组织封闭为主。结论根尖周病变影响根尖封闭成型的种类,根尖周病变严重的牙齿牙根发育受限严重,多数病例牙根不发育,仅硬组织封闭根尖,根管粗大。 Objective To analyze the root apical type and related factors effecting on immature permanent teeth after apexification.Methods Seventy five teeth had been formed root apex after apexification. The radiographs before and after treatment were saved in computer. The development and healing of the apex were compared with that of the same teeth in the another site of the same jaw. The development of root, infection around root apex, length of time for apex closure and filling of Vitapex were recorded. All data were analyzed by SPSS 10. The regression was used to selected factors effected on root apex.Results According to the Frank's observation the root apical type were divided into 3 types. Twenty five per cent teeth were type 1 that the root developed as the same pattern with the compared teeth. The regression confirmed that peri-apical infection was highly associated with root apical type. The teeth with infection had a shorter root with apical barrier.Conclusion Peri-apical infection was effected on the apical type of immature young permanent teeth. The development of root was dependent on the activity of the pulp. Control of infection was important.
