种植体周骨缺损修复后骨—种植体界面扫描电镜观察 被引量:3

The effect of bone defect size on the osseointegration of bone-implant interface

摘要 目的探讨种植体周不同骨缺损范围骨再生修复后骨-种植体界面结合情况。方法在犬股骨种植体一侧分别形成水平宽度3mm,垂直深度5mm,水平长度分别为0mm、1mm、2mm、3mm、4mm的标准梯度骨缺损,一侧覆盖生物膜后缝合,另一侧直接拉拢缝合。术后3个月处死动物,取含种植体的骨段进行扫描电镜观察,测定骨性接触率。结果水平深度为0mm、1mm、2mm、3mm骨缺损,骨与种植体直接接触均在60%以上。获得高接触率(约90%)的为1mm无膜组,2mm有膜组及3mm有膜组。4mm缺损新骨与种植体间骨的直接接触较少,有较均匀的间隙存在,无膜组间隙(约43μm)大于有膜组(6~20μm)。结论在3mm以下的骨缺损骨与种植体界面无论有无生物膜均获得很好的骨结合,此种情况下可考虑不放置生物膜。 Objective To investigate the effect of bone defect size on the osseointegration of bone- implant interface with or without guided tissue regeneration. Methods On both sides of bone walls of implants in dog' s femoral bones, bone defects, which were 3mm in width, 5mm in depth and 0mm, lmm, 2mm, 3mm,or 4mm in length were respectively made. On one side, guided tissue regeneration with collagen membrane was used; while on another side direct suture was performed. Three months after the operation, bone - implant interface of the specimens were observed with scanning electronic microscope. Results For the bone defects which length is 0mm, 1 mm, 2mm, or 3mm, all the bone - implant integration rates were beyond 60 96. The maximum bone - implant integration rate was acquired in the groups such as lmm defect in length without biomembrane covered, 2mm and 3mm defects in length with biomembrane covered. In the group of 4mm defects, bone- implant contact was seldom observed, The bone- implant space in non- membrane group( about 43μm) was larger than that in membrane - covered group ( 6 - 20μm). Conclusion For bone uefects less than 3mm in length, the bone - implant interface can acquired satisfactory osseointegration without biomembrane covered. Under this circumstance, it is not necessary to use guided - issue regeneration.
