疾病模型动物拟临床研究概念的形成及其应用——自体血栓法制备小型猪心肌缺血模型的研究 被引量:13

摘要 目的:提出疾病模型动物拟临床研究的基本概念,对自体血栓法制备小型猪心肌缺血模型进行系统的拟临床研究。方法:采用心导管介入技术,经左冠状动脉前降支注入自体血栓,制备小型猪心肌缺血模型。采用拟临床方法观察模型冠状动脉造影、30点体表心电图、无创血流动力学参数及超声心动指标,综合评价心肌缺血小型猪的病理生理特点和临床表现。结果:模型组注射自体血栓后即刻LAD造影显示血栓注射部位以下血管堵塞;造模6d后,LAD仍处于阻塞状态;体表心电图标测显示模型组心肌缺血程度(∑-ST)及心肌缺血范围(N-ST)均明显高于假手术组;造模6天后无创血流动力学检测显示模型组CO、SV、LCW、MAP均明显降低。结论:介入法自体血栓注射后成功建立小型猪心肌缺血疾病模型,并可通过疾病模型动物拟临床研究的方法得以证实。 Objective: To define the clinical-mimetic research in animal models, and to study the myocardial ischemic model in miniature swine by self-embolus under the direction of the defination. Methods: By intervention technique in present study, the myocardial ischemic model in miniature swine was prepared by injecting the self-embolus introduced into left anterior descending (LAD) coronary artery. The coronary angiography, 30 points body surface electrocardiogram (BS-ECG) , hemodynamics, ultrasonics, quantitative histology and pathohistology were determined after six days. Results: In the model group, the LAD coronary artery was basically embolized, the degree(Z-ST) and scope of myocardial ischemia(N-ST) in BS-ECG were increased, hemodynamic indexes of the cardiac output (CO) ,cardiac index (CI) ,left cardiac work(LCW) and mean artery pressure (MAP) were decreased, the cardiac muscle appeared large infarction area. Conclusion: This novel model of myocardial ischemia in Chinese miniature swine is quite close to clinical pathophysiological conditions. The success of the model can be approved by clinical-mimetic research in animal models.
