摘要
目的 探讨髂骨钉与腰骶椎弓根钉连接技术在腰骶骨盆固定中的稳定性。方法 取4具新鲜冷藏的人尸体腰骶椎、骨盆标本,依次实施3种固定术式:(1)髂骨翼拉力螺钉;(2)L5~S1椎弓根钉棒系统;(3)髂骨钉与L5椎弓根钉连接系统。对上述试件分别与无固定组进行生物力学稳定性比较。同时还对22例应用髂骨钉行腰骶骨盆固定手术的患者进行随访。结果髂骨钉在腰骶骨盆固定中的稳定性最好,各种运动状态下的活动度均明显减少,与无固定组比较差异有非常显著性意义(P〈0.01)。临床应用疗效满意。结论应用髂骨钉与腰骶椎弓根钉连接技术是维持腰骶骨盆间稳定的理想术式:
Objective To discuss the stablity of the fixation between lumbar-sacral spine and pelvis with the connectible skill using iliac screw and lumbar-sacral spine pedicle screw. Methods The lumbar and iliac vertebrae, pelvis from font fresh frozen cadaveric specimens were operated in three different ways of fixation respectively: ( 1 ) iliac ala lag screw; (2) L5 -S1 pedicle screw rod system; (3) iliac screw and L5 pedicle screw connecting system. Take three ways of fixation as mentioned above compare with the intact group in blomechanical stability. Meanwhile, 22 patients who had been treated with iliac screw for the fixation between lumbar-sacral spine and pelvis were followed-up. Results Using iliac screw for the fixation between lumbar-sacral spine and pelvis turn out to be the best in stability, the range of motion (ROM) decreased obviousely in each situation, there was significant difference between the intact group and the group calTied out by iliac screw for the fixation between lumbar-sacral spine and pelvis (P 〈 0. 01 ) . Therapeutic effectiveness is satisfied in clinical application. Conclusion Appling the connectible skill using iliac screw and the lumbar-sacral spine pedicle screw is the ideal method in maintaining the stability between lumbar-sacral spine and pelvis.
