摘要
报告了３１例胫、腓骨开放性骨折术后发生伤口感染的情况，并对其原因及防治作了初步探讨。发现术前骨科医师对小腿受伤后软组织损伤及污染程度估计不足，对骨折固定方法选择不当以及术中某些操作不当，术中、术后急于闭合污染严重的伤口等，都是导致胫、腓骨开放性骨折术后伤口感染甚至骨髓炎的原因。对胫、腓骨开放性骨折除进行严格清创外，应视其损伤原因和损伤程度对骨折选择合理固定方法，术中操作应尽量保护皮肤血运，不强行闭合伤口，同时术后应放置引流或冲洗引流。
31 cases of tibia and fibula open fracture,in which occurred post operative wound infection,are reported.The reasons,especially iatrogenic causes,are initially explored.It was found that orthopedists did inadequate estimation of the extent of soft tissue injury and contamination of legs before operation, there were improper fixed methods of fracture to be chosen,and infected wound not to be controlled were closed postoperatively in haste and so on.All of them were the reasons that tibia and fibula open fracture occurred postoperative wound infection even osteomyelitis.To cure tibia and fibula open facture,beside completely clearing wound,optimum fix methods of fracture according to the reason and extend of injury is important.Whats more,to improve the blood supply of skin,not violently close the wound and place drainage tubes or set lavage drainage is advisable.
