外伤性蛛网膜下腔出血后脑血管痉挛的临床研究 被引量:5

摘要 目的探讨外伤性蛛网膜下腔出血后脑血管痉孪与脑损伤的程度和蛛网膜下腔出血量的关系。方法选择80例外伤性蛛网膜下腔出血的患者。于入院时进行格拉斯哥昏迷评分。根据CT扫描进行改良Fisher分级,采用经颅多普勒检测脑血管痉孪,观察外伤性蛛网膜下腔出血后脑血管痉挛与脑损伤的程度及蛛网膜下腔出血量的关系。结果本组脑血管痉挛发生率为40%,随着入院后伤病情逐渐加重,脑血管痉挛的发生率逐渐升高;随着改良Fisher分级的增加,脑血管痉孪的发生率也逐渐增加。结论外伤性蛛网膜下腔出血后脑血管痉挛与脑损伤的程度和蛛网膜下腔出血量密切相关。 Objective To explore the relationship between cerebral vaso spasm after traumatic subarachnoid hemorrhage and the degree of brain injury as well as the bleeding volume. Methods 80 cases of traumatic subarachnoid hemorrhage were chosen to evaluate GCS on admission. Improved Fisher was graded according to CT scan : Transcranial Doppler was used to detect the vesospasm so to observe the relationship between cerebral vaso spasm after traumatic subarachnoid hemorrhage and degree of brain injury as well as the bleeding volume. Results The total incidence of cerebral vaso spasm was 40% ,which increased with the development of the injury. The incidence of vasospasm was gradually increased with the increase of improved Fisher grading. Conclusion Traumatic subarachnoid hemorrhage is closely related to the degree of brain injury as well as the bleeding volume.
