卡托普利对慢性肺源性心脏病中肺动脉高压的影响 被引量:2

摘要 目的探讨卡托普利对慢性肺源性心脏病(肺心病)肺动脉高压的影响及其机制。方法将60例肺心病急性加重期病人随机分为对照组(30例)和观察组(30例),对照组给予一般常规治疗,观察组在常规治疗的基础上加用卡托普利,测定两组治疗前后的三尖瓣区最大反流速度(Vmax),计算收缩期肺动脉压(PAPs);测定正常组及两治疗组治疗前后血管内皮生长因子(VEGF)含量的变化。结果肺心病急性加重期病人血清中VEGF的含量及PAPs治疗前明显高于正常组(P<0.01),观察组与对照组治疗后VEGF,PAPs均较治疗前明显降低(P<0.01),但治疗组优于对照组(P<0.05)。结论加用卡托普利治疗较常规治疗更有助于肺心病病人肺动脉高压的缓解及VEGF含量的下降,VEGF可能参与肺心病的病理生理过程,卡托普利缓解肺动脉高压可能与改善肺血管内皮功能、抑制肺血管平滑肌细胞增生,调节肺血管重构有关。
