围术期轻度低温对患者T淋巴细胞亚群免疫功能的影响 被引量:4

摘要 目的观察围术期轻度低温对颅脑手术患者T淋巴细胞亚群免疫功能的影响。方法50例ASA Ⅰ或Ⅱ级择期开颅手术患者随机分为轻度低温组(L组)和常温组(W组),每组25例。分别在麻醉前、术后第1天、第5天采静脉血1ml,用流式细胞仪测定T淋巴细胞亚群CD3+、CD4+、CD8+阳性细胞百分率及CD4+/CD8+比值。结果L组患者术后第1天CD3+、CD4+、CD4+/CD8+显著低于术前(P<0.05或P<0.01),W组患者术后第1天CD4+、CD4+/CD8+降低显著(P<0.05)。术后第5天,两组患者的各项指标均已接近术前水平。术后第1天L组CD3+、CD4+、CD4+/CD8+显著低于W组(P<0.05或P<0.01)。结论围术期轻度低温可以抑制机体的T淋巴细胞功能。 Objective To compare the effects of the normal body temperature and mild hypothermia on T lymphocyte and subsets in patients undergoing craniotomy. Methods Fifty ASA class Ⅰ or Ⅱpatients were randomly divided into group hypothermia (L) and normal temperature (W) with 25 cases each. Blood samples were taken from perpheral vein before anesthesia and on the 1st and 5th day after operation for determination of T lymphocyte and subsets CD3^+ , CD4^+ , CD8^+ and CD4^+/CD8^+ ratio. Results On the 1st after operation , CD3^+ ,CD4^+ and CD4^+/CD8^+ ratio were significantly lower in group L, CD4^+ and CD4^+/CD8^+ ratio in group W. CD3^+ , CD4^+ and CD4^+/CD8^+ in group L were significantly lower than those in group W. The values of CD3^+ ,CD4^+ and CD4^+/CD8^+ ratio were all returned to the preoperative levels on the 5th postoperative day in both groups. Conclusion Mild perioperative hypothermia can inhibit the T-lymphocyte and subsets function in patients underwent craniotomy.
