军队人员关节镜下重建前交叉韧带 被引量:2

Arthroscopic Reconstruction of Anterior Cruciate Ligament in Military Man

摘要 目的探讨军人前交叉韧带损伤患者采用自体四股半腱肌腱+股薄肌腱重建前交叉韧带的手术方法及远期疗效。方法关节镜下以自体四股半腱肌腱+股薄肌腱为前交叉韧带重建替代物,保留少许前交叉韧带残端作为定位标志物,对35例前交叉韧带损伤军人行重建术。结果术后35例膝关节活动度均恢复至正常范围,无韧带撞击现象,前抽屉试验全部阴性,Lachman试验全部小于Ⅰ度。Lysholm评分由术前的平均52.3分提高到术后的平均88.5分,差异有显著性意义(P<0.01)。大多数患者获得满意治疗效果,可继续从事日常工作。结论关节镜下自体四股半腱肌腱+股薄肌腱重建前交叉韧带是恢复膝关节稳定性较好的方法。关节镜下重建前交叉韧带是前交叉韧带损伤军人较为理想的手术方式,其创伤较小、卧床时间短、远期疗效较好。 Objective To introduce the operation method and long-term therapeutic effects of arthroscopic reconstruction of anterior cruciate ligament(ACL) with four-stranded semitendinosus muscle tendon and gracilis muscle tendon in military man. Methods The arthroscopic reconstruction was performed on 35 military man cases of ACL injury with fourstranded semitendinosus muscle tendon and gracilis muscle tendon as the substitute and the residual ACL as the orientation marker. Results The results showed postoperative normal motion range of 35 cases knee joint without ligament impingement. The postoperative anterior drawer test was negative in all the cases. The postoperative lachman test was less than grade one in all the cases. Lysholm score was increased to 88. 5 points after the operation from the preoperative 52.3 points with significant difference. Majority of the military patients can obtain good results and continue to fly. Conclusion The arthroscopic reconstruction of ACL With four-stranded semitendinosus muscle tendon and gracilis muscle tendon is a perfect operation for restoring the stability of the knee joints. The arthroscopic procedure for navy pilots is a good operation with fewer invasions,less time in bed and perfect long-term thera- peutic effects.
