应用改良导流杂交法检测103例妇女HPV基因型 被引量:11

摘要 目的:探讨改良导流杂交法在宫颈病变患者的人类乳头状瘤病毒(HPV)基因型检测中的应用价值。方法:应用改良导流杂交法检测103例宫颈癌筛查患者(宫颈癌组29例,炎症组33例和正常组41例)的宫颈细胞21种HPV基因型,并对其中的宫颈癌组同时采用杂交捕获二代系统(HC-II)行13种高危基因型HPV检测。比较两种方法检测不同HPV基因型的阳性率差异。计算用改良导流杂交法检测的各组患者不同HPV基因型的阳性率。结果:在103例研究对象中,改良导流杂交法HPV基因型检测的阳性率达46.6%,宫颈癌组、炎症组及正常组的阳性率分别为96.6%、39.4%和17.1%,各组间差别有显著性差异(P<0.005);宫颈癌患者前10位的基因型是HPV-16(16.5%)、HPV-58(7.8%)、HPV-52(7.8%)、HPV-18(5.8%)、HPV-31(5.8%)、HPV-6(4.9%)、CP8304(4.9%)、HPV-39(3.9%)、HPV-68(3.9%)和HPV-11(3.9%),而且存在HPV双重以上的感染。与HC-II检测结果的符合率达96.6%(28/29)。结论:改良导流杂交法与HC-II检测结果无差异性,可用于检测宫颈病变患者的HPV基因型。本组宫颈癌患者最常见的5种HPV基因型可能是HPV-16、HPV-58、HPV-52、HPV-18和HPV-31,且存在更多的HPV双重以上的感染。 Aim:To explore the clinical value of improved HybriMax on the genital human papillomavirus (HPV) detection. Methods: One hundred and three women for cervical cancer screening( including 29 cases of cervical carcinoma group, 33 cases of inflammation group and 41 cases of health group were selected for detection of 21 HPV genotypes by improved HybriMax. The infected rates of each HPV genotypes for each group were calculated and analyzed. And 29 cervical carcinoma group patients were detected with HC-II for 13 types of high-risk HPV. The difference of the infected rates of each HPV genotypes for the above two methods was compared. Results: The total positive rate of improved HybriMax HPV detecting is 46.6%, positive rate of HPV in cervical carcinoma, chronic cervicitis and health group is 96. 4%, 39.4%, 17.1% respectively, there are differences among three groups(P 〈 0. 005). The 10 most common genotypes of the patients with cervical cancer were HPV-16 ( 16.5% ) ,HPV-58 (7.8%) HPV-52(7.8% ), HPV-18 (5.8%), HPV-31 (5.8%), HPV- 6 (4.9%), CP8304 (4. 9%), HPV-39 (3.9%) ,HPV-68(3.9% )和 HPV-11 (3.9%), and the patients with cervical cancer had more than double infection with HPV. The accordant rate of improved HybriMax and HC-II was 96. 6% (28/29).Conclusion:Improved HybriMax has good accordance to HC-II for HPV detection. It can be used to detect HPV genotypes of thc patients with cervical cancer. Their 5 most common genotypes of cervical carcinoma women in this district are HPV-16,HPV-58,HPV-52,HPV-18 and HPV-31, and exist more than double infection with HPV.
