腰部竖脊肌表面肌电指标测量的可重复性特点 被引量:7

摘要 目的:以竖脊肌表面肌电耐疲劳性指标中位频率下降率和协调性指标屈曲伸直比为观察指标,验证表面肌电信号采集的可重复性,评价表面肌电图临床应用的实际价值。方法:实验于2006-05/07在解放军空军总医院表面肌电实验室完成。①对象:正常志愿者18名,女性10名,男性8名,年龄(30.0±8.7)岁,均签署知情同意书。②实验方案:应用芬兰MEGA公司,ME6000-T8型表面肌电测试系统进行3次测量,前2次间隔2h,第3次间隔7d,测量腰部竖脊肌疲劳性指标频率下降率和协调性指标屈曲伸直比。③测试方法:频率下降率参照Biering-Srensen测试方法,要求受试者俯卧,髂嵴上缘以下的身体固定,头端悬于检查床之外,保持脊柱伸直,手放于身体两侧,记录从躯干伸直开始到头端下垂10°肌电信号;屈曲伸直比参照flexion-extension方法,要求受试者首先直立,背靠垂直墙面,眼睛目视前方,双足与肩同宽,然后躯干缓慢前屈至最大限度,停留5s后恢复到直立体位,屈曲伸直比=屈曲过程中表面肌电平均振幅/伸直过程中表面肌电平均振幅。③统计学分析:用SPSS11.0软件分析3次数据的差异性P值和一致性指标组内相关系数ICC。结果:18名受试者均完成测试并进入结果分析。①竖脊肌屈曲伸直比:3次测量数据间差异显著性检验,左侧P=0.697,右侧P=0.532;原始数据一致性分析左侧ICC=0.979,右侧ICC=0.981,组内相关系数显著性检验P均<0.001。②中位频率下降率:3次测量数据间差异显著性检验,左侧P=0.559,右侧P=0.427;原始数据一致性分析左侧ICC=0.908,右侧ICC=0.997,显著性检验P均<0.001。结论:腰部竖脊肌疲劳性、协调性表面肌电指标频率下降率和屈曲伸直比测量的可重复性佳,表面肌电可用于腰部竖脊肌电生理功能评价。 AIM: To verify the reproducibility of collecting surface electromyography (sEMG) signals by observing the median frequency slope (MFs) of fatigue resistance and flexion-extension ratio (FER) of harmonization index during the sEMG measurement of lumbar erector spinae, and appraise the practical value of sEMG in clinics. METHODS: From May to July of 2006, the experiment was carried out in the sEMG Laboratory of General Hospital of Air Force of Chinese PLA.(1)Eighteen normal volunteers, including 10 females and 8 males, aged (30.0±8.7) years, were recruited in the study. Informed consents were obtained from all the volunteers. (2)All the subjects were examined with ME6000-T8 sEMG apparatus made by MEGA Co. in Finland, totally for 3 times, with the intervals of 2 hours and 7 days. MFs of fatigue index and FER of harmonization index were collected. (3)According to Biering-Soerensen testing method, the volunteers underwent a static EMG evaluation, for which they were asked to keep their upper body flatly while their lower body (below the superior border of iliac crest) fastened, with their heads hanging out of examining table and their hand putting on both sides of body, until upper body bending 10 degree. When examined harmonization sEMG index FER, the volunteers underwent a dynamic EMG evaluation, for which they were asked to stand, keeping advanced visuals and feet at shoulder width apart, afterwards they bended forward as far as possible, stayed full flexion for 5 seconds and then returned to standing. FER=average amplitude of sEMG during flexion/average amplitude of sEMG during extension.(4)Differential P value and intraclass correlation coefficient (ICC) of three measurements were analyzed with SPSS 11.0. RESULTS: All of 18 subjects completed the test and entered the result analysis. (1)There were significant differences in FER among three measurement data, P value was 0.697 in left lumbar erector spinae and 0.532 in right side respectively. ICC was 0.979 in left side and 0.981 in right side respectively (P 〈 0.001).(2)With the significant differences, P value was 0.559 and 0.427 respectively. ICC was 0.908 and 0.997 respectively (P 〈 0.001). CONCLUSION: Reproducibility of sEMG harmonization and fatigue indexes FER and MFs is acceptable during repeated measurements, so sEMG can be applied to evaluate electrophysiological function of lumbar erector spinae.
