摘要 目的探讨PC-Windows平台上重组活体肾移植供体肾脏及其血管立体图像的方法,在供肾摘取术前和术中对临床手术医师提供指导。资料与方法10名亲属活体肾移植供者行CT薄层扫描,图像以DICOM格式保存,在PC-Windows XP平台上读入TGS Amira3.1软件后,通过图像裁剪、分割、三维重组等步骤进行处理。结果获得了供体肾脏及其血管解剖结构的立体图像和动画演示,其结果方便其他PC机查看,并可按要求输出。合成结果显示8名供体双侧肾脏均为单支动脉供应,2名供体存在副肾动脉,均符合术中所见。结论该项技术为活体肾移植中移植肾的术前评估和术中切取提供了便利,准确性高,花费时间少,方便临床医师掌握,所需设备价格低廉,对CT工作站是一个有益的补充,临床医师可以根据实际需要灵活机动地对CT图像进行处理,更好地指导临床工作。 Objective To find a handy way to display the anatomy of the renal arterial and venous systems of potential donors on PC Windows platform. Materials and Methods Dual phase serial computed tomography (CT) images from 10 living related donors were loaded into TGS Amira 3.1 running on a PC with Windows XP. Image crop, segmentation and 3D reconstruction followed. Results We got the video clips as well as the 3D images of donor kidneys and blood vessels. According to the display which was confirmed by latter intraoperative findings, 8 donors had single renal vessels, 1 had a left accessory renal artery and 1 had a right accessory renal artery. Conclusion This project provides a new approach to evaluate the renal vessel anatomy in living donor kidney transplantation, and it is favorable for accuracy and popularizing. As the beneficial supplement of expensive imaging workstations, it is much easier to be learned and manipulated by clinicians, and the clinicians can do their own interpretations according to the raw image data, it absolutely should be favorable for the accuracy of operation.
