摘要
目的探讨颅底骨折合并口鼻腔大出血的急诊救治。方法回顾我院急诊近五年来收治25例颅底骨折合并口鼻腔大出血的急诊抢救结果。25例患者中前颅底骨折18例,前、中颅底骨折7例,均有口鼻腔大量出血,血压偏低,甚至休克。在抢救中首先以保持呼吸道通畅,同时抗休克治疗,配合行双侧鼻腔导尿管气囊压迫+碘伏纱条填塞进行有效止血。结果治愈19例,死亡6例。结论颅底骨折合并口鼻腔大出血时,病情凶险,死亡率高,在急诊中应积极救治。
Objective To study the emergency rescue of basicranial fracture complicated with massive hemorrhage. Methods To review the therapies for 25 patients of basicranial fracture complicated with massive hemorrhage in our hospital within 5 years. Among 25 patients, 18 cases were anterior and 7 cases were anterior and mid basicranial fracture, all of them had massive hemorrhage from nose . The blood pressure of the patients were low or they were in shock. In the salvage, the respiratory way was kept unobstructed, and the shock was corrected at the same time. Balloon tamponade through urethral catheter of bilateral nasal cavity ＋ indophor ribbon gauze packing were used for the effective hemostasis. Result 19 cases were cured and 6 cases died. Conclusion The patients＇conditions are dangerous and mortality is high when the patients have basicranial fracture complicated with massive hemorrhage, so we should remedy positively in emergency.
