细胞角蛋白34βE12在乳腺肿瘤中的表达及意义

摘要 目的研究细胞角蛋白34βE12(CK34βE12)在乳腺肿瘤组织中的表达情况及与乳腺癌恶性程度的关系。方法用免疫组化方法对77例乳腺良性肿瘤和135例乳腺癌组织石蜡标本的CK34βE12的表达水平进行测定,统计分析CK34βE12在乳腺良性肿瘤与乳腺癌组织中表达的差异,同时分析CK34βE12表达情况与乳腺癌肿瘤生物学行为指标之间的关系。结果CK34βE12在乳腺良性肿瘤组织中阳性表达率76.6%,在乳腺癌组织中阳性表达率44.4%,2组相比有显著性差异(P<0.05)。CK34βE12的表达与乳腺癌患者的年龄、月经状态、ER、PR等均无相关性(P>0.05);但与组织学分级、临床分期、病理类型、腋淋巴结阳性转移等指标有相关性(P<0.05)。结论与乳腺良性肿瘤相比,乳腺癌组织中CK34βE12的阳性表达率低。CK34βE12阴性表达提示乳腺癌预后不佳,可用于乳腺癌恶性程度和预后的判断。 Objective It is to study the expression of cytokeratin 34βE12 (CK34βE12) in breast tumor tissue and the relationship between CK34βE12 expression and breast cancer malignant degree. Methods The expression levels of CK34βE12 in 77 cases of breast benign tumor tissue and 135 cases of breast cancer tissue were detected with immunohistoehemistry method. The difference of CK34βE12 expression in breast fibroma tissue and breast cancer tissue was analyzed. And the relationship between CK34βE12 expression and the breast cancer biological parameter was also analyzed. Results The positive expression rate of CK34βE12 in breast benign tumor tissue was 76.6 % and that in breast cancer tissue was 44.4 %, and there was significant difference between the two groups (P 〈 0.05). CK34βE12 expression had no correlation with age, menstrual status, ER and PR (P〉0. 05), but it had correlation with histology grading, clinical staging, pathology category, axillary fossa lymph node positive metastasis ( P 〈 0.05 ). Conclusion Compared with breast benign tumor tissue, the positive expression rate of CK34βE12 in breast cancer tissue is lower. The negative expression of CK34βE12 hints worse prognosis, which can judge breast cancer malignant degree and prognosis.
