60%泛影葡胺皮下注射治疗家兔淋巴漏 被引量:3

Experimental studies of the treatment for lymphatic fistulas experimental animal model by 60 % meglumine diatrizoate subcutaneous injection

摘要 目的观察60%泛影葡胺皮下注射对家兔淋巴瘘的治疗作用。方法对90例家兔采用腹股沟部手术形成淋巴瘘模型。模型建立后第3天以60%(V/V)泛影葡胺溶液于该侧股血管走行区域分点2~3点皮下浸润注射,必要时重复注射。记录局部引流量及治疗显效率。1周后观察局部大体病理变化情况。结果术后模型成功78侧(48.1%),实验组40侧,共行60%泛影葡胺注射81次219点,对照组38侧,共行0.9%生理盐水注射107次289点,实验组注射后每日局部引流量与对照组比较明显减少(P<0.01)。注射后每日治疗累计显效率明显提高(P<0.05)。术后1周大体病理见局部血管神经组织周边粘连明显,与周围组织融合呈条索状,部分已成瘢痕样改变,两组间手术感染率差异无统计学意义(P>0.05)。结论术后淋巴瘘的发生可能与手术部位广泛分离、假腔形成、淋巴液回流量和感染等有关。泛影葡胺局部皮下注射治疗淋巴瘘疗效显著,应早期应用。重复注射可有效提高治愈率。同时具有防止淋巴管感染作用。皮下注射其优势在于费用低廉,同时局部刺激作用较轻,耐受性良好。 Objective To evaluate the treatment of lymphatic fistulas experimental animal model by 60% meglumine diatrizoate subcutaneous injection. Methods 90 experimental rabbits were underwent the groin surgery to format lymphatic fistula model. Three days later,60% ( V / V) meglumine diatrizoate was injected subcutaneous at the lateral femoral vessels running regional by 2 -3 points,when necessary to repeat vaccination. Record the local daily drainage and treatment efficiency significantly. Observe the local pathology changes after one week of operations. Results 78 of 180 laterals (48.1%) model were formated successfully ,the experimental group including 40 sides,held a total of 60% meglumine diatrizoate injection 219 points of 81 times, the control group was 38 sides,held a total of 0.9% saline injection of 289 points of 107 times. After injection, the experimental group shows local daily drainage volume compared with the control group decreased significantly ( P 〈0.01 ). After injection of a total daily treatment sign/ficantly increased their efficiency ( P 〈 0.05 ). The pathologic review shows local vascular peripheral tissue adhesion obviously, and the surrounding tissue fusion was strip-shaped, the region has become part of scarlike change. Surgical infection rates between the two groups no significant difference ( P 〉 0.05 ). Conclusion Lymphatic fistula may occur with widely separated of surgery,false cavity formation,lymph flow and the infection. Meglumine diatrizoate injected for lymphatic fistula treatment efficacy significantly, and should been applied earlier. Repeat injections are effective in improving the cure rate and can prevent lymphatic infection. Its advantage lies subcutaneous injection of low-cost, local stimulation lighter, good tolerability.
