摘要
目的研究气囊上滞留物间歇吸引降低机械通气患者早期呼吸机相关肺炎(ventilator-associated pneumonia,VAP)的作用。方法选择机械通气超过48 h的患者58例,随机分为气囊上滞留物间歇吸引组(观察组,n=28)和普通人工气道护理组(对照组,n=30)。比较两组患者VAP的发生率和气囊上滞留物的含菌量。结果观察组气囊上滞留物含菌量显著低于对照组(P<0.05);观察组1周内VAP发生率(7.1%)低于对照组(33.3%),差异有统计学意义(P<0.05)。结论气囊上滞留物间歇吸引可降低声门下气囊上区含菌量,降低早期VAP的发生率。
Objective To study the effect of intermittent subglottic secretion drainage (ISSD) on the incidence of early-onset ventilator-associated pneumonia (VAP) in patients receiving mechanical ventilation. Methods Fifty-eight patients with mechanical ventilation for over 48 h were randomized into 2 groups to receive ISSD with an endotracheal tube (ISSD group, n=28) or routine airway care (control group, n=30). The incidence of VAP was recorded and the bacterial content in the subglottic secretions were measured in each patient, and the results were compared between the two groups. Results The bacterial content in the subglottic secretions in ISSD group was significantly lower than that in the control group( P〈0.05). During the initial week of observation, VAP occurred in 2 patients (7. 1%) receiving ISSD,a rate significantly lower than that (10 patients,33.3G) in the control group( P〈0.05). Conclusion ISSD reduces the bacterial level in the subglottic secretions and the incidence of early-onset VAP in patients receiving mechanical ventilation.
