摘要
目的:评价布托啡诺用于全麻术后镇痛的效果及安全性。方法:选择经全麻手术的、ASA分级为Ⅰ或Ⅱ级的患者90例,随机分为布托啡诺组(n=45)和吗啡组(n=45)2组,术毕前30min给予负荷量,布托啡诺组予布托啡诺0.01mg/kg,吗啡组予吗啡0.05mg/kg,在麻醉清醒后静脉接韩国产奥美2300型微量持续电子镇痛泵,给药模式均为负荷剂量+持续剂量2mL/h+单次剂量2.5mL(锁定时间15min)。其中布托啡诺组术后镇痛泵配方为布托啡诺0.2mg/kg+甲氧氯普胺15mg,吗啡组为吗啡1mg/kg+甲氧氯普胺15mg。对2组镇痛效果(VAS评分、BCS评分、Ramsay评分)和并发症进行观察分析。结果:2组镇痛效果差异无显著性意义(P>0.05),但布托啡诺组术后24h平均动脉压及呼吸频率与吗啡组比较差异有显著性意义(P<0.05),总不良反应发生率明显低于吗啡组(P<0.05)。结论:布托啡诺用于全麻术后镇痛效果满意且不良反应发生率低于吗啡,值得临床推广使用。
Objective：To evaluate the effect and complication and safety of Butorphanl or Morphine combined with Metoclopramide for postoperative＇analgesia in patients undergoing general anasethesia operation. Methods：90 ASA Ⅰ or Ⅱ patients undergoing general anaesthesia operation were divided into two groups randomly. 30 minutes before completion of the operation,loading dose was giren：in Butorphanl group,0.01mg/kg;in Morphine group,0. 05 mg/kg. After anaesthesia awakening,Automed 2300 micro continous electron analgesia pump run per vein, and administration mode was： loading dose gas given first,continuous dose Was 2 mL/h,single dose（PCA） was 2.5 mL,loeked time was 15 mins. Butorphanl group,45 cases applied with Butorphanl 0. 2 mg/kg and Metoclopramide 15 mg. In Morphine group,45 cases applied with Morphine 1 mg/kg and Metoclopramide 15 mg. Observation, analysis and complications of the two groups（ including measure the VAS, the Ramsay, the BCS Scale mainly）were observed and analyzed for analgesia effect. Results ：There was no significant difference between the two groups in anlgesic effeet（P 〉 0. 05 ）, but there was significant difference in the mean arterial blood pressure（MABP） and respiratory rate （RR） in Butorphanl group and total adverse reaction incidence rate in Butorphanl group was obviously lower when compared with that of Morphine group 24 h after operation（P 〈0.05 ）. Conclusion：The effect of Butorphanl in postoperative venous analgesia may be as satifatory as that of Morphine,but it causes less adverse reactions than Morphine. So it is of practical clinical significance.
