绝经后女性急性冠脉综合征患者性激素水平与树突状细胞亚群变化 被引量:1

Estradiol,Testosterone and Dendritic Cell Subsets in Patients of Post-menopause Women with Acute Coronary Syndrome

摘要 目的评价绝经后女性急性冠脉综合征(ACS)患者外周血雌二醇(E2)、睾酮(T)水平与树突状细胞(DC)前体各亚群分布情况及相关性。方法ACS患者62例为实验组,其中急性心肌梗死(AMI)20例,不稳定型心绞痛(UA)22例,稳定型心绞痛(SA)20例,对照组为胸痛综合征(CPS)10例,非冠心病10例,用化学发光免疫分析法测定外周血E2、T水平,流式细胞仪四色分析法检测外周血单核细胞(PBMC)中髓样DC(mDC)和浆细胞样DC(pDC)亚群占比例(%),计算其绝对数(个/μL),评价各组之间的差异,并对ACS组内各项指标进行相关性分析。结果ACS患者外周血E2水平,E2/T比值,mDC与pDC占PBMC的比例、绝对数及其比值均较CPS和非CHD组低(P<0.01,P<0.05);E2水平与mDC所占PBMC比例、绝对数及与绝对数的mDC/pDC呈正相关(r=0.408,r=0.622,r=0.590;P均<0.01);E2/T与mDC所占PBMC比例、绝对数及其mDC/pDC呈正相关(r=0.498,r=0.604,r=0.633,P均<0.01)。结论绝经后女性E2水平降低,E2与T之间平衡失调,影响了对免疫反应的调控,可能与ACS的发生、发展有关。 Objective To evaluate estradiol(E2)and testosterone(T) level, dendritic cell (DC) subsets in peripheral blood in patients of post-menopause women with acute coronary syndromelACS). Methods This cohort of patients consisted of 62 patients with ACS 120 cases with AMI, 22 cases with UA, 20 cases SA), 10 cases CPS, 10 cases without CHD as controls. The level of E2 and T were determined by chemiluminescent immunoassay; the proportional number of myeloid dendritic cell I mDCt, plasmacytoid dendritic cell lpDC) were measured by flowcytometery. Results ACS patients had lower E2 level, lower ratio of E2/T, lower percentage of mDC and pDC in peripheral blood monouclear cells(PBMC). The absolute number of mDC, pDC and mDC/pDC were lowered in patients with CPS in relative with patients without CHD(P〈0.01, P〈0.05 ). E2 level and E2/T were positively correlated with mDC, absolute number and it's mDC/pDC(all P 〈0.01 ). Conclusion The decreases in E2 level and the imbalance between E2 and T in acute coronary syndrome in post-menopause females were associated with decreases in dentric cells suggesting that E2 take part in the pathogenesis and development of ACS.
