摘要
目的:探讨合并改良电抽搐治疗(MECT)并发精神病性症状的抑郁症患者的临床疗效及安全性。方法:80例并发精神病性症状的抑郁症患者随机分为MECT组及对照组各40例,均给予抗抑郁剂治疗,MECT组加用MECT治疗;对照组加用利培酮治疗。治疗前后均行汉密尔顿抑郁量表(HAMD)、简明精神病量表(BPRS)及副反应量表(TESS)评定。结果:治疗第1周末时,MECT组HAMD总分及各因子分、BPRS总分与治疗前比较均明显下降,对照组在治疗第2周末时开始下降;第4周末时,MECT组显效率为75%,对照组为67.5%,2组显效率相近。TESS分值2组比较差异无显著性意义。结论:MECT治疗并发精神病性症状的抑郁症起效快,改善症状早,且安全。
Objective: To study the clinical effect and safety of modified electro-convulsive treatment (MECT) in combination with antidepressants for depression associated with psychotic symptoms, Methods: Eighty depression patients associated with psychotic symptoms were randomly divided into MECT group and and control group (n=40 in each group). Both two groups were treated by anti-depressive drugs. The patients in MECT group were subjected to MECT and those in control group were treated with risperidol. All patients were assessed by HAMD, BPRS and TESS before and after treatment. Results: The total scores and the scores of every factor of HAMA and BPRS were significantly reduced in MECT group at the end of first week after treatment as compared with those before treatment; In control group, the total scores and the scores of every factor of HAMA and BPRS were significantly decreased at second week after treatment. At the end of 4th week after treatment, the excellent effective rate was 75% and 67.5% in MECT group and control group respectively. There was no significant difference in TESS between two groups. Conclusion: MECT is safe and effective in treating depression associated with psychotic symptoms.
