摘要
目的测量不同水平甲状腺激素人群颈动脉内-中膜厚度(IMT),并探讨其与甲状腺激素的关系。方法运用彩色多普勒超声测量100例甲状腺功能异常患者颈动脉IMT,包括40例甲状腺功能亢进症患者、30例甲状腺功能减退症患者、30例亚临床甲状腺功能减退症患者,并以40例甲状腺功能正常人群作为对照组。同时测定被检查者的血压、空腹血糖、血脂以及甲状腺激素[包括游离T3(FT3)、游离T4(FT4)、超敏促甲状腺激素(TSH)]。IMT与甲状腺功能进行简单直线相关分析。以颈总动脉IMT为因变量,以年龄、血压值、空腹血糖值、总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、甲状腺激素等为自变量,进行多元线性逐步回归分析。结果各组IMT比较差异有显著性,IMT与FT3,FT4,TSH无直线相关(r值分别为0.12,0.06,-0.39,P<0.05)。多元线性逐步回归分析显示TSH与IMT独立相关(回归系数B=-0.176,P=0.024)。结论IMT与甲状腺功能无直线相关。TSH是IMT的独立影响因子。
Objective To investigating the relationship of IMT with thyroid hormone by detecting carotid artery intima-media thickness of thyroid dysfunction patients, Methods One hundred thyroid dysfunction patients including 40 hyperthyroidism patients, 30 hypothyroidism patients, 30 subclinical hypothyroidism patients and 40 normal subjects as control were analyzed. The carotid IMT was measured with color Doppler ultrasound. The blood press,blood glucose, lipoprotein and thyroid hormone (including free T3 (FT3) ,free T4 (FT4) ,TSH)were detected. Linear correlation analysis was performed to test the relationship between the thyroid hormone and carotid IMT. The association between age, blood press, blood glucose, serum total cholesterol(TC), triglyceride(TG), high - density lipoprotein cholesterol(HDL- C), LDL cholesterol(LDL- C), thyroid hormone and IMT was assessed with multiple regression analysis. Results There was significant difference in IMT of all groups. There was no linear relationship between IMT and thyroid hormone ( r = 0.12,0.06, -0.39, respectively, P 〈 0.05). Multiple regression analysis showed there was an independent association between TSH and IMT( B = - 0.176, P = 0.024). Conclusion There is no linear relationship between IMT and thyroid function. TSH is an independent impact factor for IMT.
