摘要
目的:目前肩关节周围炎的治疗多以非甾体类抗炎药、免疫抑制剂、激素等药物治疗为主。由于口服药物存在较多的副作用,在一定程度上限制了使用和该病的治疗效果。观察红外线配合青鹏膏剂对肩关节周围炎治疗的有效性及其应用中出现的不良反应。方法:①试验对象:选择2006-10/2007-01在解放军第二军医大学长海医院和上海市市东医院康复医学门诊就诊的肩关节周围炎患者70例,诊断参考中国人民解放军总后勤部卫生部《临床疾病诊断依据治愈好转标准》,患者对试验及治疗均知情同意,治疗方案经医院医学伦理委员会批准。采用区组随机化方法将70例患者分为治疗组(n=30)和对照组(n=40)。②试验方法:治疗组在患处外涂青鹏膏剂的基础上再给予红外线治疗,对照组给予单纯红外线治疗。③试验评估:利用视觉模拟评分法和肩关节活动功能评分进行疗效分析,同时观察治疗过程中有无不良反应发生。结果:纳入肩关节周围炎患者70例,均进入结果分析。①对照组和治疗组治疗1周后疼痛均有改善,但两组差异无显著性(P>0.05);治疗后2周治疗组优于对照组(P<0.05)。②治疗后两组肩关节活动功能的改善基本一致,治疗1周和治疗2周的结果差异无显著性意义(P>0.05)。③治疗组患者外用青鹏膏剂,未发生局部皮肤瘙痒、破溃等不良反应。结论:红外线配合青鹏膏剂具有能迅速减轻肩周炎患者疼痛的作用,但在改善肩关节活动功能方面没有突出的效果。
AIM: Medication such as non-steroid anti-inflammatory drug, immunosuppressant and hormone is the common therapy for periarthritis of shoulder. However, the side effects of oral-taken medicine limit the application and curative effect to certain extent. This study observed the effect and adverse reaction of infrared rays and Qingpeng paste therapy on periarthritis of shoulder.
METHODS: ①Seventy cases of periarthritis of shoulder admitted in Changhai Hospital and Shanghai East Hospital were selected from October 2006 to January 2007. The diagnosis was accorded with Clinical Diagnosis Norm of Cure and Improve of Disease, Ministry of Health of General Logistics Department. The informed consent was obtained from all patients, and the treatment was approved by the Hospital Ethics Committee. The subjects were randomly divided into treatment group (n =30) and control group (n =40). ②Infrared rays were given to the control group. Qingpeng paste therapy was used for the treatment group under the basis of infrared rays. ③Visual analogue scale and Range of motion were used to observe the curative effect. Side effect was observed at the same time.
RESULTS: Seventy patients with periarthritis of shoulder were all involved in the result analysis. ①After one week treatment, the pain of both groups was improved with no significant difference (P 〉 0.05). After treatment for two weeks, the improvement in treatment group was superior to control group (P 〈 0.05). ②The range of motion of shoulder joint improved in both groups, and the differences were not statistically significant after one and two weeks treatment (P 〉 0.05). ③There was not any side effect such as itch of skin or ulceration in the treatment group.
CONCLUSION: Qingpeng paste combined with infrared rays can relive the pain caused by periarthritis of shoulder. However, it is not remarkably effective in improving the range of motion of shoulder joint.
