摘要
[目的]探讨HBsAg阴性血清中是否有HBV-DNA的存在,以对我国目前献血员的乙型肝炎筛查安全性进行初步评价。[方法]利用酶联免疫吸附试验对HBsAg阴性的1430例非献血员和1608例献血员血清进行乙肝标志物检测,剔除单独anti-HBs阳性和全阴性血清,其余血清采用聚合酶链反应定量检测HBV-DNA拷贝数。[结果]剔除单独anti-HBs阳性和全阴性血清后,非献血员标本中其他阳性组合模式有52份(3.64%),HBV-DNA呈阳性1份(0.07%);献血员标本有182份(11.32%),HBV-DNA呈阳性4份(0.25%);所有HBV-DNA阳性血清均为anti-HBc阳性。[结论]HBsAg阴性血清中仍有HBV-DNA存在的可能,仅检测HBsAg筛查献血员是不够的,应对献血员进行更严格的HBV标志物检查,例如增加anti-HBc的检测。如有条件,可检测HBV-DNA。
[Objective]To explore the existence of the HBV-DNA in the serum,so as to preliminary assess the screening safety of HBV among blood donors in our country.[Methods]The HBV markers of 1 430 blood non-donors and 1 608 blood donors were detected,and the serum samples with positive anti-HBs and all the negative HBV marks were rejected. The HBV-DNA copy numbers of other serum samples were detected by using polymerase chain reaction.[Results]After rejecting serum samples with positive anti-HBs and all the negative HBV marks,52 serum samples with other positive integrated modes(3.64%)and one serum samples with positive HBV-DNA(0.07%)were found in the serum samples of blood non-donors;182 serum samples with other positive integrated modes(11.32%)and 4 serum samples with positive HBV-DNA(0.25%);All the serum samples with positive HBV-DNA revealed to be positive anti-HBc.[Conclusion]Serum samples with negative HBsAg could find HBV-DNA,and it is not enough to detect the HBsAg of blood donors. Strict detection of HBV markers should be performed among blood donors,such as adding the detection of anti-HBc. If it is possible,HBV-DNA should be detected.
