摘要 目的:探讨125碘(125I)粒子组织间放射治疗过程中医护人员的辐射安全。方法:利用便携式辐射探测仪分别监测22例恶性肿瘤患者植入125碘粒子整个过程中操作人员手掌、胸部及眼睛等部位的照射量率,然后通过相关公式计算相应部位的年剂量当量,最后依据国家标准GB18871《电离辐射防护与辐时源安全基本标准》进行安全性评估。结果:通过对治疗过程的监测和换算,医护人员手掌、胸部和眼睛等部位无屏蔽状态下年剂量当量最大值分别为(222.45±11.7)mSv/a、(12.75±1.2)mSv/a和(25.5±1.65)mSv/a,而在进行有效屏蔽时年剂量当量最大值则明显降低,分别为(30.26±2.55)mSv/a、(0.45±0.06)mSv/a和(0.84±0.09)mSv/a,远远低于国家标准GB18871《电离辐射防护与辐时源安全基本标准》中的年剂量当量限值。结论:125碘粒子组织间放疗对医护人员不会造成确定性的辐射危害,是安全的、可接受的。但仍需要重视潜在照射的危险性。在开展该治疗方法时要严格控制适应症。 objective: To investigate radiation safety during interstitial radiotherapy of iodine-125 (^125I) seeds. Methods: Twenty-two cancer patients treated with ^125I seeds interstitial implantation therapy were studied. All of them were given thirty-nine ^125I seeds. Measure the relevant received dose of the operators' palms, chest and eyes by the portable dose rate-meter during ^125I seed interstitial implantation therapy. Then base on these measurement data, to calculate the dose equivalent of body with some specified formulas. Finally, base on the dose equivalent and national standard (GB18871) to evaluate the operators' radiation safety. Results: The maximal dose equivalent of operators' palms, chest and eyes without any shield, are(222.45±1 1.7) mSv/a,(12.75±1.2)mSv/a and(25.5+1.65) mSv/a. The maximal dose equivalent of the operators decreased obviously while under the certain shields, are (30.26±2.55) mSv/a,(0.45±0.06) mSv/a and (0.84±0.09) mSv/a. These dose equivalents are very lower than the upper limit of the national standard. Conclusions: The interstitial seed implantation therapy will not bring any certain radio-trauma,and it is safe and acceptable even without any shield. But the risk of potential exposure should be regarded.We should lay down operation indications and avoid performing operation blindly.
