摘要
目的探讨精索静脉曲张(VC)与精浆中性α糖苷酶(NAG)活性和果糖水平的关系。方法将170例男性不育患者根据有无VC及精液参数正常与否分为VC精液参数异常组(A组)、无VC精液参数异常组(B组)、VC精液参数正常组(C组);将37例无VC且精液参数正常者设为正常对照组。根据是否合并其他致不育因素,再将A组和B组进一步分为不合并其他因素组(A1组和B1组)和合并其他因素组(A2组和B2组)。检测并比较各组精浆NAG活性和果糖水平。结果A组和B组NAG活性均显著低于正常对照组(P=0.000),但A、B两组间差异无统计学意义(P=0.107)。C组与正常对照组相比,NAG活性无统计学差异(P=0.245),A组NAG活性较C组显著降低(P=0.000)。A1组NAG活性显著低于B1组(P=0.028)。各组间果糖水平差异均无统计学意义(P>0.05)。结论NAG活性减低可能是VC导致男性不育的原因之一,而果糖水平并不受VC的影响。
Objective To investigate the relationship between the activity of neutral alpha-l, 4-glucosidase (NAG) and the amount of fructose in the semen of patients with varicocele. Methods The activity of NAG and the level of fructose were measured in the semen of 170 infertile males. The subjects were divided into the following three groups: group oligoasthenozoospermia with varicocele (group A), group oligoasthenozoospermia without varicocele (group B), group normozoospermia with varicocele (group C). Thirty-seven cases of fertile men with normal seminal parameters were served as controls. Group A and group B were further divided into subgroups depending on whether there was a detectable cause (subgroup A2 and subgroup B2) or no detectable cause ( subgroup A1 and subgroup B1 ) for their abnormal seminal parameters. Results The NAG activity of group A and group B were significantly lower than the controls (P = 0. 000) , while there was no significant difference between group A and group B (P = 0. 107). There was also no significant difference between group C and controls (P =0. 245). Group A had lower levels of NAG activity compared to group C (P =0. 000), and subgroup A1 had lower levels of NAG activity compared to subgroup B1 (P = 0. 028). There was no significant difference in the amount of seminal fructose among all groups (P 〉 O. 05). Conclusion Varicocele-related infertility may be caused by the lowered activity of NAG in semen. However, the amount of fructose do not appear to be related to the occurrence of varicocele.
