摘要
目的:观察正常妊娠中、晚期孕妇血液流变学参数的变化。方法:运用CF型围产监测仪检测左室射血阻抗、动脉特性阻抗、动脉终端阻抗及总外周阻抗,以研究外周阻力变化;检测毛细血管血流速度及微循环灌注连续性比率,探讨微循环改变;同时探讨血容量与血粘度变化。结果:妊娠后5~10个月左室射血阻抗、动脉特性阻抗、动脉终端阻抗及总外周阻抗较对照组明显减少(P<0.01),各月间上述指标无明显差异(P>0.05);孕妇毛细血管血流速度明显加快(P<0.01),至妊娠10月达高峰;毛细血管灌注连续性比率则明显下降(P<0.01),以妊娠10个月最低。该期间内,孕妇血容量明显增加(P<0.01),血粘度在孕5个月时最低,以后渐回升,至孕10个月时已恢复正常水平。结论:正常妊娠中、晚期孕妇血管外周阻力降低,微循环功能增强,血容量增加,血粘度以妊娠中期最低,以后渐增加,至足月时恢复妊娠前水平。
Objective:To observe the hemorrheologic changes of normal pregnancy. Methods: With the help of CF type peripartal detective and diagnostic instrument, ventricular ejective resistance(VER),arterial specificity resistance(ASR),arterial terminal resistance(ATR) and total peripheral resistance(TPR) were observed to investigate the changes of peripheral resistance;capillary flow velocity (CFV) and circulatory continuity ratio (CCR) were monitored to study the changes of microcirculation;and the changes of circulatory blood volume(CBV) and dynamic blood viscosity(DBV) were also investigated. Results: During the middle and late pregnancy,all of the VER,ASR,ATR and TPR greatly decreased (P<0.01) , while there were no significant differences among each month. CFV greatly increased with the pregnant month and reached to the highest level in the tenth month(P<0.01). On the contrary,CCR greatly decreased (P<0.01). CBV was elevated with the months of pregnancy ( P< 0.01) , while DBV decreased considerably and reached the lowest level in the fifth month, then it increased gradually and returned to the normal level in the tenth month. Conclusion: During the middle and late pregnancy, the peripheral resistance decreased, the function of microcirculation and CBV increased, while DBV decreased in the early and middle stage and reached to the lowest level in the fifth month, then it increased gradually and returned to the normal level in the late stage.
