有限元分析全环与半环型外固定架的力学稳定性差异 被引量:7

摘要 目的比较全环与半环型外固定架的力学稳定性差异,为骨科临床选用骨折固定装置提供参考。方法运用三维有限元方法对现有的全环和半环式外固定架的轴向和侧向稳定性进行分析以判断二者各自的适用范围。然后对全环外固定架的各项参数进行生物力学优化。结果在力学性能上,半环固定架与全环Ilizarov外固定架存在较大差异。Ilizarov全环外固定架无论是轴向抗拉压强度和抗侧向压力强度都较半环式外固定架高许多。外固定架的稳定性随着固定圆环个数、圆环间距、克氏针夹角的增加而增加,当克氏针夹角达到90°时固定效果最好。结论全环外固定架的力学特性要高于半环外固定架,但半环外固定架的装卸更简单、方便,其选用要结合临床。外固定架安装时应该尽可能增大固定圆环个数、圆环间距和克氏针夹角。 Objective To compare the difference of mechanical stability between whole-ring external fixator and half-ring external fixator and provide evidence for fixator choosing in clinic. Methods Three-dimensional Finite Element Method was adopted to analyze the axial and lateral stabilities of both whole-ring and half-ring external fixator for determining their own applicability. Then various parameters were optimized through the results of Finite Element analysis. Results Whole-ring external fixators were much more stable than half-ring ones in mechanical stability including axial and lateral stabilities. The stabilities of ring external fixators increased with the increase of ring number,distance between rings and angles between two transfixing wires. And the fixators were the most stable when the angle reaches 90°. Conclusion Whole-ring external fixators are more stable than half-ring ones in mechanical stability. However,the latter one is more convenient during handling. The clinical application of the fixators should combine the analysis of physical signs and the degree of bone impairment. The fixators should be assembled with larger distance between rings,more rings and largest angles to great extent.
